Fault/ Defect Title: Faulty Air Vent & Isolation gate Valve not installed

Fault/ Defect Description: Corrective Maintenance (CM) reported by Station Master (SM):
192411: Heavy water leakage in firefighting pump room coming from CHWR
PIPE as per reported by SG patrolling.

When ACIFM inspected, it was found that water leakage is due to faulty air vent
installed on Chilled water pipes of FCU. It was also noted that there is no isolation
valve installed to isolate the air vent.

 For temporary mitigation, isolation valve was closed on the CHWS &
CHWR and opened the bypass valve. Unit was kept in Maintenance
mode

JV shall replace the defective air vent and also installed the missing PSH JV 27.08.2020
isolation valve before the air vent and shall make the unit in normal
operational condition

Root cause analysis shall be conducted on the above-mentioned defect, PSH JV 27.08.2020
corrective action shall be derived and implemented
