Fault/ Defect Description: During the asset conditional survey, the following accessibility restrictions to the
equipment were observed.
There is no access to Extract fan (TEF) motor & blower for the maintenance
activity. WCE is installed the above the AHU and its duct. However, there is no
place to put ladder and to erect scaffolding as well as to open the access door of
fan MEP & BACS tag is different and MEP tag is not installed on the unit. Refer
to the below photos.
Actions Taken: • Visual inspection done and noticed that unit has inaccessible for the
maintenance activity.
• Asset is classified into Access category- E1, which is used and
recorded in the Asset Conditional Survey. Definition of access
category is mentioned in supporting information.
JV shall demonstrate the accessibility strategy, for the identified MEP PSH JV 22/02/2021
equipment in a safe way, relocation of unit or other services shall be done if
required to enable a safe access way to the unit to carry out maintenance
activities and install the life-line to connect the harness for safety.
Root cause analysis shall be conducted on the above-mentioned defect, PSH JV 22/02/2021
corrective action shall be derived and implemented
