The hoist must be in @ safe condition to use and this must be taken in to account when deciding whether to accept or reject the equipment, Ensure SWL and identification number is legible and is cross-referenced on the current proof load report oF current examination report. Examine for: ‘+ Bottom and upper safety limits for corect operation. * Loose, missing or distorted bolts. All washers, including locking washers where appropriate, and locking devices must be in place. All moving parts for slackness, wear and other damage. © Damage or distortion to the slack end anchor that connects the load chain to the block casing and/or signs of damage to the load chain stop (where one Is fitted). ‘© Chain guide or stripper for wear or damage. * Load wheel is not worn or damaged and that the chain seats snugly into the pockets of the load wheel. * IFaslack chain collection box's fitted make sure that it s securely attached and that the drainage holes are clear. * Hoist stops when the operating button/pendant controller is released. * Cracks at bends or changes in section. Mechanical damage, nicks, gouges and corrosion to the hoist body. Chain for the opening of the linkages and that it is securely fastened. + Alllinkage parts on the twist control for security and ease of operation. DT = 800 N/mm?— power driven hoists used in abrasive conditions. 31 monthly examinations are required for fing accessories and for ‘equipment used to lift persons. The industry's practice is to examine all lifting equipment at 6 monthly intervals. 41 Where no defects are noted and the hoist can continue to be used safely then the report is to be issued to the client within 28 days, Form No LD-001 is issued when a single examination is conducted and Form No. LD-002 is to be issued when examinations have been conducted on a number of items of li ing equipment. The information required in both reports is the same. 5) Where defects are noted that need to be corrected within a specified time scale the report should be issued to the client promptly, normally on, the day the examination has been carried out. In the UK a report issued under these conditions is to be sent to the Health and Safety Executive. ‘Comments Location Identification markings In the absence of specific manufacturer's technical information on the chain's permissible wear, then the following is to be used: Examine links for wear 8% reduction of the original diameter being the maximum permitted. The identification number. safe working load, range of lif and name of manufacturer is to be permanently marked on the hoist, This will be in the form of a plate or tag permanently attached to the hoist. The following Information is also to be available: - year of manufacture, required air pressure and delivery rate and details of the chain size and grade. This information will be available in the operating manual. Report/ceriification Prior fo the thorough examination, the inspector must sigh! either the. current test report or the last examination report in order to confirm that the hoist has been tested and/or examined correctly and to familiarise himself with any special conditions applied to the Iifting equipment. The Yorious types of reports that may be available are as follows ‘+ Report of a test and thorough examination * Report of a thorough examination * Declaration of conformity PART4 SAFETY CONSIDERATION AND RISK ASSESSMENT ‘Safely considerations The appropriate work permit is to be raised prior to any examinations being commenced. The area where the test is being conducted Is to be restricted to necessary personnel only. The adequacy of floors, passages and landings are to be assessed before test weights are moved or placed. Personnel are to wear appropriate personal protective equipment related to the work site, Duting the inspection of the hoist, the shut-off vaive should be closed. Safety hamesses are to be used when working at height and when necessary. Risk assessment When conditions or the work site constitute the need for a risk assessment, the following should be used for guidance:- * Look for the hazard. '* Decide who might be harmed, and how. ‘Evaluate the risk and decide whether existing precautions are adequate. * Record the findings for future reference. * Review the assessment and revise (if necessary).
