I am writing to formally inform you that I will be resigning from my post as a Customer Service
Representative of the company due to the reason that I am starting my internship at Benguet
General Hospital to complete my required units for this semester.

My internship schedule is conflicting with my day shift schedule at work, though I have tried to
make adjustments but to my dismay, it was not successful and I was forced to conclude on this
decision.

Having been given the opportunity to be a part of the company is such an honor for me.Thus, I
would like to express my gratitude to the management who has given me the guidance and
support that I needed to perform my job properly for the past years.
