MOU for LABORATORY SERVICES

This Reference Laboratory Services Agreement (“Agreement”) is made
effective this _20_ day of __November__ 2022__ (the “Effective Date”), by and between
Alnoor Lab Habibabad and Family surgical Hospital Habibabad (Client)

WHEREAS, CLIENT is a professional healthcare provider that maintains a good clinical

patient history has identified a need to obtain clinical laboratory diagnostic testing services from a
reference laboratory; named Alnoor Lab Habibabad which is registered with PHC.
Labs which are done locally will be sent to alnoor lab but sensitive tests like harmones and
histopathology will be arranged by alnoor lab to their parent lab.
Results. LAB shall make available the results of Laboratory Services to CLIENT
(“Results”). The parties agree to cooperate in good faith with respect to any applicable reporting
obligations. LAB and hospital both shall be responsible for reporting to applicable health
authorities in case of danjerous diseases like HIV.

CLIENT RESPONSIBILITIES.

Hospital will send all labs to alnoor laboratory in the form of indoor collection.

FINANCIAL ARRANGEMENTS.

Finances will be met by hospital on mutual benefits.

Confidentiality.Neither party will disclose confidential or proprietary information relating
to the operations (such as strategic plans, business plans, financial information, volume

data, etc.,) belonging to the other party that has been identified as confidential or
proprietary or is otherwise not generally known to the public, except as consented to or
requested by the party owning such information or as required by law.

IN WITNESS WHEREOF, the parties have executed this Agreement effective as of the
Effective Date first written above.
