MEMORANDUM OF UNDERSTANDING FOR ICU SERVICES

This agreement is made between, _SATYAM HOSPITAL AND RESEARCH CENTRE (Name of Hospital
first party), an establishment under Clinical Establishments Acts with registration number
______________ having its office at 26, Pokaran House , Near JDA Circle, Loko Road Ratanada
Jodhpur Rajasthan 342001(address of Hospital first party) hereinafter referred to as _SATYAM
HOSPITAL (which expression shall unless it is repugnant to the context or meaning thereof shall deem
to mean and include its successors and assignees)

And

GOYAL HOSPITAL AND RESEARCH CENTRE (Name of Hospital second party with ICU services) having
its office 961/3 RESEDENCY ROAD SARDARPURA JODHPUR 3430021 (address of Hospital second
party) hereinafter referred to as GOYAL HOSPITAL which expression shall unless it is repugnant to the
context or meaning thereof shall deemed to mean and include its successors or assignees).

WHEREAS, SATYAM HOSPITAL (first party) is an MULTISPECIALITY HOSPITAL (speciality of Hospital)

hospital providing various services to their patients which includes amongst them insured individuals
and groups, corporate, self-funded groups, foreign travellers and insurance companies and for this
purpose SATYAM HOSPITAL (first party) has created a network of services providers.

SATYAM HOSPITAL (first party) has approached the GOYAL HOSPITAL (second party) to provide
medical services (of which they are capable of) to the beneficiaries at a reasonable cost.

GOYAL HOSPITAL (second party) agrees to provide the necessary services on the terms and
conditions, herein mentioned:
SCOPE OF SERVICES

1. GOYAL HOSPITAL (second party) is ready to provide services for sick ICU, NICU & CCU
patients as well as to all referred patient for critical care services.
2. GOYAL HOSPITAL (second party) will provide critical care services to patients send by
SATYAM HOSPITAL (first party) whenever required.
3. GOYAL HOSPITAL (second party) will provide 24 hours critical care services to patients
referred by SATYAM HOSPITAL (first party).
4. GOYAL HOSPITAL (second party) will provide professional in courteous manner whenever
patients will be referred for tertiary care as applicable for all referred patients.
5. GOYAL HOSPITAL (second party) will provide discount to poor patients as per policy of the
hospital and when required & applicable.
6. Effective date of agreement is _01-04-2024 and valid upto 31-12-2027

Non Exclusivity

SATYAM HOSPITAL (first party) reserves to appoint any other centre for required services and GOYAL
HOSPITAL (second party) will have no objection as in case currently.

Jurisdiction

All disputes are subject to Jodhpur jurisdiction only.

By signing below each party agrees to the terms of this agreement.
