Performance Checklist: Peripheral Lab Draw Using a Straight (Eclipse) Needle and a Vacutainer Holder
II. Gather all needed equipment and supplies
Alcohol prep pad
2X2 gauze pad
Tourniquet
Tape
Clean gloves
Label(s)
Biohazard bag
Straight (Eclipse) needle
Vacutainer holder
Vacutainer tubes
III. Perform Entry Measures
IV. Choose Site
Assess for Contraindications:
History of mastectomy
Removal of lymph nodes
Presence of AV fistula
Presence of PICC line
V. Prepare
1. Open gauze pad
2. Prepare tape
3. Attach Vacutainer holder to Eclipse needle
4. Put on clean gloves
VI. Performance
1. Apply tourniquet~4 inches above site using quick release knot. Be sure not
to leave on for more than 1 minute.
2. Clean site for 30 seconds with alcohol prep pad and allow to dry (do not
blow or fan site).
3. Twist off needle cap and insert needle at 15‐30 degree angle.
4. Insert tubes into the Vacutainer device in correct order of draw
5. Remove tourniquet.
6. Withdraw needle and activate needle safety device.
7. Cover site with 2 X 2 gauze pad, and apply pressure to site.
8. Discard needle in sharps container.
9. Label tube(s) at bedside with date/time/initials and site.
