FAMILY
NURSING CARE
PLAN
SURVEYED BY: BAGAY, JUSTINE KATE H.
DATE: SEPTEMBER 15, 2021
HEALTH PROBLEM 1
Nature of the
Constipation related
Problem
to inadequate fluid 2/3 x 1
intake = 0.67
JUSTIFICATION
This is considered a health threat because there is a failure to
realize one’s health potential and to maintain the wellness of each
member of the family. Especially that they are not drinking enough
water when, in fact, water helps with everything. Since about 60%
of the human body is actually 7 water, it needs to maintain
hydration levels to fuel cells and keep the brain and body
functioning. (Baum, I. 2022).
HEALTH PROBLEM 1
Modifiability of
Constipation related
the Problem
to inadequate fluid 1/2 x 2
intake =1
JUSTIFICATION
Because the nurse is familiar with this condition and has current
knowledge for constipation, this is partially modifiable. Additionally,
the community has resources that can be used to treat this illness.
But the family's compliance is the only issue. We don't know whether
they will cooperate if we, as nurses, educate them about what
should they do.
HEALTH PROBLEM 1
Preventive
Constipation related
Potential
to inadequate fluid 3/3 x 1
intake =3
JUSTIFICATION
If constipation is not being prevented, there might be a
severe or long-lasting constipation that may cause or
raise the risk of various health issues, including: Anal
fissures are tiny rips inside the anus. Hemorrhoids are
bulging, painful veins in the region of the lower rectum and
anus.
HEALTH PROBLEM 1
Salience
Constipation related
0/2 x 1
to inadequate fluid =0
intake
JUSTIFICATION
It is not perceived as a problem as evidenced
by, “Normal na sakin ‘to mangyari. Maselan
kasi tyan ko tsaka after ng dalawang araw
nawawala naman agad.”
HEALTH PROBLEM 1
Constipation related TOTAL SCORE:
to inadequate fluid
2.67
intake
HEALTH PROBLEM 2
Nature of the
Disturbed sleeping Problem
pattern 2/3 x 1
= 0.67
JUSTIFICATION
The results revealed that short sleep duration might
result in low hemoglobin concentration and that disrupted
sleep also raised the risk of anemia. Poor sleep associated
to changes in sleeping patterns can have a negative impact
on health. (Liu et al, 2018)
HEALTH PROBLEM 2
Modifiability of
Disturbed sleeping the Problem
pattern 1/2 x 2
=1
JUSTIFICATION
The nurse is aware with this situation and may provide health education.
Furthermore, there is now recognized information on having sleep
problems, which is why this is partially modifiable. Additionally, the
community contains resources that may be utilized to address this
condition, including the ability to provide appropriate medicine. The main
difficulty is with the family's cooperation, however. Whether nurses
instruct them on what they should do, we don't know if they will comply.
HEALTH PROBLEM 2
Preventive
Disturbed sleeping Potential
pattern 2/3 x 1
= 0.67
JUSTIFICATION
If the condition is properly addressed as soon as necessary,
symptoms such as increased sensitivity to stress, physical
discomfort, decreased quality of life, emotional distress and
mood disorders, as well as deficiencies in 9 cognition,
memory, and performance, are minimized or even eradicated.
HEALTH PROBLEM 2
Salience
Disturbed sleeping 1/2 x 1
0.5
pattern
JUSTIFICATION
The client recognized the problem but not
needing immediate attention evidenced by “Hindi
nga tama ang ganitong sleeping pattern ko pero
kasi masayang mag fb at manuod ng kdrama pag
gabi tapos hindi rin ako makatulog.”
HEALTH PROBLEM 2
Disturbed sleeping TOTAL SCORE
pattern 2.84
HEALTH PROBLEM 3
Nature of the
Imbalanced Nutrition Problem
More Than Body Requirements 3/3 x 1
associated with food intake that =1
exceeds body needs as evidenced
by BMI.
JUSTIFICATION
It presents a deficit to one's health since the prevalence of
overweight people is alarming and because these conditions are likely
to develop if a person consumes more food than their body can utilize
for energy. It has long been thought to be responsible for important
other health issues. Although it is not the ideal method for assessing
cardiovascular risk, the risk of heart disease and stroke rises as BMI
rises. (Wayne, G. 2022)
HEALTH PROBLEM 3
Modifiability of
Imbalanced Nutrition the Problem
More Than Body Requirements 1/2 x 2
associated with food intake that =2
exceeds body needs as evidenced
by BMI.
JUSTIFICATION
The registered nurse is aware of the problem and may provide guidance on how to
maintain a balanced diet as part of her role as a health educator. In addition,
there is now well accepted information on obesity, which means that this may be
easily modified. In addition, there are resources available in the community that
may be used in the treatment of this condition. These resources include the ability
to give appropriate medicine and treatment. In addition, if the patients
participate in the health education, it will be possible to monitor their progress
since our nurses will be keeping a log of their actions.
HEALTH PROBLEM 3
Preventive
Imbalanced Nutrition Potential
More Than Body Requirements 3/3 x 1
associated with food intake that =1
exceeds body needs as evidenced
by BMI.
JUSTIFICATION
If the condition is handled, it is possible to prevent cardiovascular
disease, type 2 diabetes, sleep disturbances, infertility in women,
increased musculoskeletal problems, and shorter life expectancy. This
implies that if you are overweight, you are more likely to have these
illnesses, which place you at a higher risk of developing cardiovascular
disease.
HEALTH PROBLEM 3
Salience
Imbalanced Nutrition 1/2 x 1
More Than Body Requirements = 0.5
associated with food intake that
exceeds body needs as evidenced
by BMI.
JUSTIFICATION
The family recognized this as a problem but not needing
immediate attention as evidenced by “Sinabi nga ng
doctor na overweight daw ako kasi hindi nag tutugma
height ko sa pounds ko. 11 Pero hindi ko naman na
napagtuunan ng pansin. Nag try ako mag gym noon pero
tinigil namin dahil nagkaroon ng problema".
HEALTH PROBLEM 3
TOTAL SCORE:
Imbalanced Nutrition
More Than Body Requirements 4.05
associated with food intake that
exceeds body needs as evidenced
by BMI.
