o Completed Membership Form

o Photocopy of one (1) Valid ID of the Business Owner with three (3) signatures
o Certified True Copy of Business Permit or Barangay Permit
o Worth Php 20,000.00 accumulated GUGMART Receipts

QUALIFICATIONS

o The following businesses are qualified to be part of the GUGMART BUSINESS MEMBERSHIP
PROGRAM:
 Wholesale/Retail of Grocery Items such as:
o Sari-Sari Store
o Mini Grocery
o Drug Store
 Canteen, Catering, Food Service, Food Stall, Restaurant
 Other businesses in line with the above mentioned
o Members must be at least 18 years of age.
o Minimum Purchase is Five Thousand Pesos (Php 5,000.00)

o Call – Order – Pick-up Service via call/text/e-mail
o Wireless and COD Transactions
o GUGMART BUSINESS PARTNER CARD. NO CARD, NO TRANSACTION POLICY.

PROGRAM PERIOD
o This program starts on December 1, 2022 and is valid until December 31, 2023.
