CHÁNH: What is orthopedics department. Does it provide bone and joint care?

DƯƠNG: Yes. Orthopedics is hospital department that specializes in the treatment of


bones and muscles.

CHÁNH: I see. Where’s that department?

DƯƠNG: It’s on the third floor. Turn left when you pass the radiology department. You
can find that department between pathology and dermatology department.

CHÁNH: Great. I will head up there now.

DƯƠNG: But first you should refill your prescription at the pharmacy.

CHÁNH: The pharmacy is downstairs, next to the emergency room, right?

DƯƠNG: You’re right. You can also use the wheelchair at the back of this room.

C: I’m here to have a full check about my asthma.
D: OK. But first please go to radiology to take your chest X-rays. It’s on the third floor
next to pathology department.
C: Sure. I will be back soon.
C: Hello doctor here is my test results.
D: Hmmm. I have taken a look at your chest X-rays.
C: Yes. What did you find doctor?
D: I’m so sorry to say, it appears to be emphysema.
C: Oh really? I thought it was my asthma.
D: Well, you’ve had asthma for years. But this is an entirely new condition.
C: As I know asthma is a chronic disease of the lungs. It involves inflammation of the
bronchial tubes and other airways. This causes wheezing, coughing and difficulty
breathing. So what is emphysema.
D: Emphysema is a progressive respiratory disease. It is the result of bronchial tube
damage. In emphysema, the alveoli lose the shape and functionality when the person
exhales. As a result, less oxygen is allowed to enter the bloodstream. Smoking tobacco is
one of the most common causes of emphysema.
C: That doesn’t sound good. Can it be cured?
D: Unfortunately, emphysema is incurable. But there are treatments that can slow its
progressive.
C: I hope they work. I just want to breathe well.
D: Please take medicine on time and call me if you have any problem about breathing.

C. I came here because I had an abdominal pain.

C. Yeah. It’s the same pain as a week ago. I have had persistent pain for two days. At first
it was a dull pain. Lately the pain has become sharper. I’m a little worry about it. Is it that
serious?

D. All right. You know, we’re probably going to have to do colonoscopy.

D. Colonoscopy is a medical procedure. That will check your colon and intestines for any
serious problems.

D. Your blood pressure is 150/90. It’s a bit too high. You are at increased risk for heart
diseases.

C. What are some common heart diseases.

D. That’s lots common heart diseases like coronary artery condition as known coronary
thrombosis, heart failure, heart problem at birth (some children are born with heart
defect) and especially heart attack.

C. So far as I know, heart attack is a serious medical emergency when something blocks
the blood flow to our heart. Heart attacks are a leading killer of both men and women, is
that right?

D. Each year, more than 1M people in the US have a heart attack, and about half of
them die. Half of those who die within 1 hour of the start of symptoms and before
reaching the hospital.

C. Could you explain what happens during a heart attack?

D. Certainly. Usually, an artery inside the heart is called coronary artery suddenly gets
blocked by fatty acids or clots.

C. That doesn’t sound good.

D. No. it’s not. When that happens, blood can’t get through. That can lead to permanent
damage or death.

C. Oh my God. What are the systems?

D. Sudden chest pain or difficulty breathing. Call the115 immediately and ask for an
ambulance if you have or suspect someone has a heart attack.

C. I will. I should stop smoking and do regular exercise, eat a low-fat food to prevent a
heart attack. Thank you doctor.

D. Mr. An should receive 200 mg of heart disease pills.

D. PO would work better. He should take before meal with water, three time per day.

C. How about Mr. Bao. He was complaining that he had a sharp abdominal pain. What
should we do about this.

D. Sure. Mr. Bao can have paracetamol if he asks for it, but no more than 500mg per
day.

C. Okay. He will want to take that orally, right?

D. No. He can’t swallow the pills. He is also vomiting when he tastes something bitter.
He should take fluids through an IV.

C. I see. Thanks doctor. Have a good night.

C. Okay Doctor. Hmm, he was admitted to the hospital with serious dehydration and
breathing trouble, so what does he need?

D. Firstly, for his dehydration he should take glucose 5% through an I.V.

C: Do you think it’s enough for him? I think he needs steroids.

D. He is also needs a 200mg dosage of steroids. The route of administration was an


intramuscular injection (IM).

C. Sure. And how often should he get those?

D. They should be taken TID until he gets better.

C. I understand Mr. An persistent illness has been caused by an infection. Serious


complications have arisen. So does he need antibiotic?

D. He can also have antibiotic pills TID, but no more than 2 tablets per day. Preferably
pc, after he takes meal.

C. Got it. I’ll add it to his chart.

Chánh: My left forearm really hurts.

Dương: I see. Where exactly does it hurt?

Chánh: All over, but especially the wrist.

Dương: Okay. Do you know what happened to make it hurt?

Chánh: I was playing volleyball and while I tried to hit the ball across the net, my wrist
twisted.

Dương: Yes. That’s likely the problem. Does it hurt in the upper arm or anywhere else.

Chánh: Just my wrist. But I feel pain through my left hand.

Dương: I think it’s just wrist sprain, but we are going to take some X-rays to make sure.

Chánh: Doctor. I have a question. What is wrist sprain?

Dương: Wrist sprains are common injuries. A wrist sprain occurs when the strong
ligaments that support the wrist stretch beyond their limits or tear. 

Chánh: I see. But where can we take X-rays, Dr.? The radiology, right?

Dương: Sure. It’s on the third floor. Turn left when you pass the pathology department.
You can find that department between pediatrics and obstetrics departments.

Chánh: Thanks Dr, I’ll head up there right now.

Dương: After you get the test result. Please be back here again.

C: My neck really hurts. Please have a look at it.

D: Hmm. Let me see, that’s likely the problem. What happened to make it hurt?

C: You know I work in a company as an office worker. I have to work with computer
almost of my work time. So I have to sit for a long time and neck become pained.

D: Okay. Seat for a long time can cause neck pain. Pain located in the neck is a common
medical condition. So any pain near the shoulder blades or chest?

C: No, that doesn’t hurt.

D: I see. Does it only hurt when you turn around your head?

C: Yes. It also hurts when I wake up or after I finish my work.

D: Okay, I think you may have a neck pain.

C: I thought so too.

D: To make sure we are going to take some x-rays to check any problems.

C: Ok Dr. Thanks for your help.

C(nurse): Hi, Dr. Duong. I just saw the patient in room 1 . She was admitted to the
hospital by a bicycle accident.

D(doctor): Hmmm. How is he doing?

C: She has several facial injuries. Most are scrapes.

D. Where exactly are the scrapes. Does she have head trauma. Can you describe more
detail about her injuries.

C: The patient has no severe head trauma. There are minor scrapes on the face and a
small bruise on her upper jaw. There are also scrapes on her left cheek near the left ear.

D. I see. Is that all. How about her forehead, eyes and nose.

C: The patient has one deep cut on the forehead. There are two minors cuts between
the right eye and eyebrow. The nose appears bruised and the upper lip is bloody and
swollen.

D. That sounds seriously. She may need stitches. I will take a look and she may take CT
scan to make sure her head is still doing good.
