THIS FORM MUST BE COMPLETED PRIOR TO THE ISSUANCE OF LEAVING CERTIFICATES.

For Primary and Secondary Students: No leaving documents will be released until a completed
“Clearance Form” has been returned to Registration Office.
Your child is expected to collect a “Clearance Form” from the Registrar’s Office which he/she will return
all of the books and gets the teachers signatures.

“Clearance Form” is to be returned to Registration Office in order to collect the Leaving

Documents.

Preparation of documents takes 5-7 working days. Please ensure that all supporting documentation
is attached.
