Cancer Center Journal

Day 1
It was November 21, 2022, I woke up at exactly at 5 am, ready to start my day. I finished
getting ready and boarded a jeepney at 5:50 am Going to the Tanchuling Cancer Center at
Legazpi City from Guinobatan. I arrived and grouped up with my classmates at 6:40 then was
called by our Clinical Instructor who was Mrs. Melissa Sue Pecson. When we arrived at the
premises, She briefed us on what to expect and to do on our duty here at the Cancer Center,
and asked us to partner ourselves up and discuss among the partners the order in when who
goes first up to last. My partners is Student Jonhel Morcoso and Jay Ar Miraflor, we were three
because our group has eleven members so one group will have three members. We stayed at
our quarters waiting for our patients. It was roughly on 1:05 pm when our patient has arrived
and we were called by our CI to take the Vital signs. We greeted Ourselves to the patient and
explained that we would take His Vital signs. I Took The patients PR, RR, Temperature and
Spo2, Jay Ar Took the patients Blood Pressure and Jonhel was getting information and
establishing rapport on our patient. While Finishing on our Vital signs taking Our CI Called us
because we would be preparing and administering the patients Pre Meds. There was three
Ampules to be prepared. The Pre Meds-Consisted of Ondansetron, Dexamethasone, and
ranitidine. Each of us took one Ampule, I got the ampule of Dexamethasone, prepared it and
Administered It Through Slow IV Push. After that we Charted the vital signs, we were asked to
take the vital signs again at 2:00 pm. While waiting We Observed our fellow classmates and
The Staff Nurses Do their Jobs. When It was 2:05 we took The Vital Signs again. After that I and
some classmates stayed in the Room where the patients are getting treatment because some
of the patients was asking for assistance, some of my classmates assisted a patient going to
the bathroom, while I assisted in a patient in standing up because as verbalized by the patient
“nawawara ang sanit pag naga tindog ako” so I helped the patient in standing up because he
was having a hard time standing up, he said he has a hard time moving his left leg. Me and
another classmate would help him up until the end of our shift. When it was about to turn
3:00 We said Goodbye to the patients and the patient that we were helping and said that if
the patient wants to stand up again the guardian can request a personal on the nursing
station. We gathered up our things and said our goodbye to our CI and each other then went
home.
Day 2

It is November 22, 2022 I woke up again at 5:00 am, prepared my things and myself and left
my house at 5:40 am, when we arrived at the cancer center we were told that my group would
be the first one to get a patient this day, when the patient arrived we learned that this patient
will be having his Blood transfusion, so we took his vital signs, after taking his vital signs prior
to the blood transfusion, we were tasked to check the blood if it was the right blood type and
serial number, after that we were taught about the filter of the blood transfusion set that was
going to be used for the patient. Jay ar was the one who inserted the Blood transfusion set on
the Y port, after doing that we charted the Vital signs we got prior to the Insertion of the
Blood. After waiting for 15 minutes we took Vital signs again to check if the patients has any
allergic reactions or any changes to the Vital signs after intaking the blood. after that we
stayed the patient side just in case if something happens. While in the room I watched my
classmates interacting with their own assigned patients. It went like this up until the all the
blood in the bag was gone, I was supposed to be the one who will take out the IV needle but
the staff nurse already removed the needle. And we took the last vital signs after a while. We
charted the last vital signs then said goodbye to our patients. We packed our things and said
goodbye to our Ci.

Day 3

I woke up at the same time as before, prepared my things and left my home at 5:55 and
arrived at the cancer center before 7 am. We waited for our CI to assign us to a patient, when
out CI finally called us to out assigned patient we, took her vital signs, and after that, there was
a doctor that was taking blood away from the patients body because the patients blood is too
much and is forming blood clots. After monitoring our current patient, another patient was
assigned to us. We took her vital signs and after that I sat by the second patient to provide
company since the second patient didn’t have any one accompany her. I chatted with her, she
showed me pictures of her grandsons. The nurse did an IV insertion on the second patient and
after a few minutes we prepared and administered through sivp. After administering I
interviewed the patient with the intention of using the patient for our case presentation, I
asked the patient to tell her story, and she began telling her story from the very beginning to
the present time, after interviewing the patient we continue our chat until the end of our shift.
I said goodbye to the patients, packed my things and said goodbye to our CI.
Day 4

Woke up at 5, had breakfast, showered and dressed myself. I left my house and boarded a
jeepney ride at 6 am and arrived before 7 am to the cancer center. We put on our ppe and
waited for our patient, while waiting we were discussing that we cannot use the one we
previously interviewed because of the lack of information, so we decided that we were going
to use our next patient as the patient for our case presentation. When that patient arrived, we
greeted the patient and did her vital signs. We couldn’t take her blood pressure since she has a
Porthacath on her left side and she was taken blood samples on her right arm, we put 120/80
since she said that she took her blood pressure prior to going to the center and 120/80 was
the result. After taking her V/S we witnessed how the staff nurse maintain the sterile field and
how to insert into a Porthacath. After that minutes passed then we were asked to prepare her
pre meds. We prepared them much better than before since we were used now to preparing
medications, after preparing we went to the patient, explained the purpose of the drug, and
administered it. After that we interviewed the patient, after interviewing, the 2 of us put the
V/S on the patients chart then we encoded what we learned from the patient. after that
nothing eventful happened, we monitored the patient and stayed in the area in case she needs
assistance. This went on until it was 2;45 pm, when we took her V/S again and put it on the
chart. After that we said goodbye to our patients, packed our things, and said goodbye to our
CI.
