Goal #1: Activities and Timeframes

How am I going to achieve my goal? Identify a minimum of three (3) learning activites with timeframes/timelines.

1. I will circle out the important information about the patient, for example, diabetes and medication administration
time, as soon as i received the report from the nurse before each shift.

2. I will prioritize the head-to-toe assessment and vital check on every shift and nish them before 7:30 for one
patient, or 8:00 for two patients.

3. During each debrief session, I will communicate with my clinical instructor about my overall performance on
that shift. I will also re ect on my practice at the end of each shift by considering which task I should complete
earlier to minimize the potential issues.

Goal #2: Activities and Timeframes

How am I going to achieve my goal? Identify a minimum of three (3) learning activites with timeframes/timelines.
1. I will volunteer to observe other nurses perform post-mortem care and try to get involved when there is a
chance throughout the 16 shifts this semester.

2. I will search for resources about end-of-life care and how to better support the patient and their family during
the process. I will review this information and try to apply them during my practice throughout the term.

3. I will invite my clinical instructor to observe me doing/mimicking post-mortem care and provide feedback on
potential improvement throughout the term.

Goal #3: Activities and Timeframes

How am I going to achieve my goal? Identify a minimum of three (3) learning activites with timeframes/timelines.
1. I will keep on practicing the skills I have done in the previous placement, and minimize the mistakes I make
during the process throughout the term. For example, gather all the required supplies in one go, and follow the
steps without skipping or switching any of them.

2. I will help my peers when performing any nursing cares requiring more than one person on each shift, and
provide them with suggestions if I am familiar with the skills, or learn from their performance if it is an unfamiliar
skill to me.

3. I will be more con dent in volunteering to try new skills, such as dressing changes, removing an IV, collecting
specimens and so on throughout the term, and re ecting on how I can do better next time during each debrief
session.

How do my learning goals support my commitment to continuing competency?
