And I hereby declare that I –
(a) have not at any time been declared a bankrupt;
(b) have not been convicted in Malaysia or elsewhere of a criminal offence which would render
me unfit to be a member of my profession, and in particular, but not limited to, an offence
involving fraud of dishonesty;
(c) do not hold concurrent practicing certificate elsewhere other than in Sabah.
I also state that I have paid the following fees and dues which are due and payable to the Law Society for
the year…………….;
(i) annual subscription in the sum of RM500.00 payable pursuant to subsection 13C(1) of
the Advocates Ordinance;
(ii) **contribution to Discipline Fund in the sum of RM200.00 payable pursuant to
subsection 12J(2) of the Advocates Ordinance; and
(iii) **contribution to Compensation Fund in the sum of RM50.00 payable pursuant to
subsection 16E(2) of the Advocates Ordinance.
I hereby apply for the Annual Certificate in order that I can present it to the Registrar to enable me to
obtain a Certificate to Practise in respect of the year........................................... and I enclose herewith the
following documents:
1. Accountant’s Report in respect of the period not earlier than 30 June prior to the year for which the
Annual Certificate is being applied for (ending at 30 June at the earliest) in the form prescribed under
the Advocates’ Account Rules 1988 or evidence that I fall within the exemption under the provisions
of subrule 16(1) of the Advocates’ Account Rules 1988;
2. A copy of the previous Accountant’s Report submitted for renewal of the Certificate to
Practise/Annual Certificate, if applicable;
3. Statutory Declaration made pursuant to sub-rule 16(1) of the Advocates’ Account Rules 1988
declaring, among others, that I did not give any discount to anyone except to permitted persons; and
I shall ensure that I am in possession of an Annual Certificate and Certificate to Practise at all times
especially when pupils are undergoing pupillage. Should I at any time for whatever reason not have an
Annual Certificate and Certificate to Practise, I shall inform my pupil accordingly to enable him to make
the necessary arrangements to continue his pupillage with another master.
I shall comply with all the requirements as set out in the Advocates’ Account Rules 1988 and Advocates
(Remuneration) Rules 1988.
