Diet History – a comprehensive record of eating-related behaviours and foods a person

eats

Contents of Diet History of Adults:


1. Check appetite – good, poor, any factors that affect appetite, taste and smell
Perception

He has a good appetite on eating foods and even the perceptions of taste and
smell.

2. Ask for allergies, intolerances or food avoidances – foods avoided and reasons,
length of time of avoidances

He has no allergies but he need to avoid eating a lot of monggo and goat dishes
because it can affect his gout and he can’t walk.

3. Consider ethnic and cultural background – eating habits and food


preferences and religion

He was a Roman Catholic, he can eat whatever he want but it needs a limitation

4. Food preference = spicy foods, sweetness of sugar and the savouriness of salt. High
fat foods, curry spices of Indian cuisine.

He like to eat spicy, sweet, soury and salty foods. But he doesn’t like to that have
curry spices or Indian Cuisine

5. Habits = choosing food out of habit = ex. Eating cereal every morning or eating a
familiar food makes them comfortable

Drinks coffee and eating bread for breakfast

6. Evaluate dental and oral health- problems w/ eating, foods that cannot be eaten,
Problems w/ swallowing, salivation

He has no problem on dental and oral because he go to his dentist monthly for
further check up.

7. Consider economic status – income, amount of money for food each week or month.

He’s income was 24k per month

8. Evaluate physical activity level:

- Occupation (type, hours/week, shift, energy expenditure), Call Center Agent, 8 hrs
but always a night shift
- Exercise(type amount, frequency) none
- Sleep ( hours/day ,uninterrupted?), sometimes 8 hrs uninterupted
- Handicaps. None
9. Determine home life and meal patterns:

- Number in household (eat together?) 6 members of the family in one household


- person who buy food: sometime the parents but sometime we can buy our own
food
- person who does cooking? Mother, Father and Eros (me, the interviewer)
- food storage and cooking facilities (stove, refrigerator): refrigerator
- type of housing (home, apartment, room): family house

10. Assess gastrointestinal conditions:


- problems of heartburn, bloating, diarrhea, vomiting, constipation,
frequency of problems, antacid, laxative or other drugs.

Sometimes he experience heartburn and bloating if he ate too much

11. Consider presence of chronic disease:


- treatment, length of treatment, dietary modification including physician
prescription, date of modification, education, compliance w/ diet.

Since his favorite was bean dishes or monggo and goat dishes he can also eat but
he will need to take Colchecine to treat his gout after.

12. Evaluate recent weight change – loss or gain, how much, over what length of time,
intentional or non-volitional

He gain weight too fast but so hard to lose weight.
