11 GORDONS FUNCTIONAL HEALTH PATTERNS

I. HEALTH-PERCEPTION-HEALTH-MANAGEMENT PATTERN

My client considers health as an absence of disease so once a person doesn’t have any disease he/she
can live a really good life. According to him we need to exercise , eat adequate amount of food, drink
vitamins and our maintenance in order to manage health. He feels that his body's condition is 70%
good since he feels okay most of the time and 30% bad because of his chronic disease which is
diabetes. With his age and current health condition his only form of exercise is sweeping the leaves
on his garden in order to sweat each morning ,as much as he wants to jog around the neighborhood
his feet can’t handle it. The reason why he got type 2 Diabetes Mellitus it’s because of his eating
habits when he was young . He used to like eating foods with high sugar content such as ice cream ,
leche flan and halo-halo . There’s no day before that he will miss eating ice cream. As of the present
he fully realized the risk of unhealthy diet after he was diagnosed with diabetes so he makes sure that
he’s really careful on what he is eating and been avoiding foods that aren’t good for him which is
what his doctor advised him. He had colds a couple times this year but it doesn’t really last for long
and he also had a minor slip last week because his feet is kind a weak already. There are rare times
when he felt dizzy and he feels like his sugar is too high so he makes sure to drink his medication in
order to manage it. His general health had been good this past weeks except during the days when
his arthritis attacks. Aside from drinking his prescribed medications he also drinks herbal medicines or
drinks for diabetes and vitamins to keep him in good condition . He also stopped having vices a long
time ago just for his health because he thinks that what is important right now is good health and
being not to sick will be helpful to the family because it won’t add up to any problems existing at
home.

II. NUTRITIONAL-METABOLIC PATTERN


⮚ Describes pattern of food and fluid consumption relative to metabolic need and pattern indicators of local
nutrient supply.
⮚ Includes the individual’s patterns of food and fluid consumption: daily eating times, the types and quantity
of food and fluids consumed, particular food preferences, and the use of nutrient or vitamin supplements
4 times a day ,fish ,coffee, talong, centrum vitamins
• Describes breastfeeding and infant feeding patterns.
• Includes reports of any skin lesions, ability to heal, and measures of body temperature, height, and
weight.
He really avoids having lesions because it will took a long time to heal and sometimes it gets worst
• General appearance of well-being and condition of skin, hair, nails, mucous membranes, and teeth are
included.
• Has dark foot, hair is greyish white, has good nails if cleanse a couple of teeth was broken

• Appetite? Sometimes he loses appetite if he’s sugar is too high

foods,

III. ELIMINATION PATTERN

⮚ Describes patterns of excretory function (bowel, bladder, and skin).


Once a day poop , sometimes not, 10 times a day , sweaty if hot
⮚ Includes the individual’s perceived regularity of excretory function, use of routines or laxatives for bowel
elimination, and any changes or disturbances in time pattern, mode of excretion, quality, or quantity of
elimination. Dapat once a day mag poop and then may urine is not normal
⮚ Also included are any devices used to control excretion

IV. ACTIVITY-EXERCISE PATTERN

⮚ Describes pattern of exercise, activity, leisure, and recreation.


Maglinis ng garden ang way of exercise everyday tapos manood ng tv or sleep the rest of the day
⮚ Includes activities of daily living requiring energy expenditure, such as hygiene, cooking, shopping, eating,
working, and home maintenance.

and compensations, dyspnea, angina, or muscle cramping on exertion, and cardiac/pulmonary classification, if
appropriate) are also included
He wants to jog around the area but he can’t because his feet can’t handle it since he has diabetes and this
disease affected his neurons on his feet which made it numb

V. SLEEP-REST PATTERN

• Describes patterns of sleep, rest, and relaxation.


There are times when he finds it hard to sleep so sometimes he
sleeps really late, before sleeping he makes sure someone will apply
linements on his and on his foot to avoid the attack of his arthritis
there’s also times when the weather is too cold his feet is greatly
affected by it since it will hurt badly
• Includes patterns of sleep and rest/relaxation periods during the
24-hour day.
Sleeps- 12pm – 7 am . Siesta 1 -2pm
• This pattern includes perception of the quality and quantity of
sleep and rest, perception of energy level after sleep, and any
sleep disturbances, as well as aids to sleep such as medications
or nighttime routines that the individual uses.
Sleep for energy , mahina ang energy , If he thinks about
problems,

VI. COGNITIVE-PERCEPTUAL PATTERN


• Describes sensory-perceptual and cognitive pattern.
Hindi mabasa ang maliliit na letters , makasolve pa ng math
• It includes the adequacy of sensory modes, such as vision,
hearing, taste, touch, and smell, and the compensation or
prostheses currently used
Okay both taste and smell ,have prostheses sa teeth ,sense of
touch on feet is medyo minimal
• Reports of pain perception and how pain is managed are
included when appropriate.
If may pain there’s a problem with the body and we should manage
by applying topical pain relievers

• Also included are cognitive functional abilities such as language,


memory, judgment, and decision making
He’s in right mind to decide all things, sometimes he forgets things
but it rarely happens

• Describes self-concept pattern and perceptions of mood state.

abilities (cognitive, affective, or physical), body image, identity,
general sense of worth, and general emotional pattern.

but he doesn’t really mind it because for him minding these
things will cause him stress and he doesn’t want that

health related.
