CASE RELATED TO ETHICAL VIOLATIONS IN THE COMMUNITY NURSING

CASE
Student Y was given the task of giving the last injection to the Ramadhan client room 2 class
1 who had diarrhea 5 days ago and was declared by the doctor who treated him to go home,
but student Y went to class 2 room 1 with the child's name Ramdhan, after being given
medicine to the client a few minutes later had convulsions and bluish. The students were
confused, they didn't know what to do, because there was no nurse on duty, so in the end,
Ramdhan's client could not be helped.

IDENTIFICATION OF PROBLEMS
a) An error was made by the nurse in charge by giving the last injection authority for the
Ramadhan patient to student Y.
b) Student Y did not pay attention to the senior nurse's order to give the last injection to a client
who had diarrhea for 5 days.
c) The people involved in this case are the nurse in charge, student Y, the nurse on duty, the
Ramadhan client and the Ramdhan client.
d) There were omissions and errors in the room, the name of the client and the administration
of the injection.
• Ramadan client --> Ramdhan
• room 2 class 1 --> class 2 room 1
e) This happened because the students did not communicate with the patient before the
injection (data validation) and during the injection the nurse on duty did not accompany
him.
f) The nurse on duty should always accompany student Y when exercising authority because
student Y is in the learning stage.
g) As a result, there was an error in giving the injection to Ramdhan's client resulting in
convulsions and blueness.
h) There was no coordination between student Y and the nurse on duty before taking the
injection.
i) Student Y was confused after a seizure and turned blue and there is no emergency action.
j) The nurse on duty did not accompany and was not available.
k) Ramdhan's client is helpless.
l) Ramadhan's client, who should have been able to go home after receiving the last injection,
ended up getting delayed.

SOLUTION

• Ensure the room and give the injection to the right client.
• The nurse on duty should accompany student Y in the nursing intervention that is carried
out.
• Student Y should be able to take emergency actions to save Ramdhan's patients by asking
for help from a nurse or doctor in another room.
• For the nurse who is responsible for carrying out each intervention, the nurse must provide
good and correct service. In this case the nurse on duty must always be in place to provide
guidance to student Y.
• The existence of client discomfort due to illness, nurses should provide services according
to standards so that clients can find coping mechanisms for their illness.
• It is the duty of nurses to provide emergency services as a humanitarian duty, when a patient
Ramdhan experiences seizures and turns blue due to the wrong injection. But it was not
done.
• The client's right to refuse treatment and its consequences are ignored. Judging from the
absence of communication to the client before the last injection.
• The nurse must consider the client's rights. In this case, it is student Y who should make the
decision due to his carelessness. The decision must consider the client's rights and comfort
by asking for help from a nurse or doctor in another room to immediately help the patient.
• Nurses must be responsible and accountable for every action they take. What has been done
and there is negligence, student Y must be ready to be responsible and accountable for his
own actions.
• The nurse in charge needs to get several warnings or forms of coaching according to the
level of their respective ethical violations. The nursing code of ethics can be applied because
the credibility of the nurse on duty is questioned. Meanwhile, student Y must be ready to be
held accountable for his actions to be brought into the realm of law.
