2. LATEST 1X1 ID PICTURE WITH WHITE BACKGROUND

Dili 2X2, Passport ID or other sizes .1X1 rajud dapat (4pcs)

3. BIRTH, MARRIED and DEATH CERTIFICATE (2 photocopies)

Dapat ang e submit kay sakto nga Certificate. If separated dapat nay Married Cert nga naka annotate na void na
ilang kasal in short dapat legally separated . If dili legally separated kay mo pirma jud dapat si spouse..dili pwede nga
single ila e declare kung married jud sila. If wrong spelling ilang Birth Certificate or married cert vs ID’s and other
document dapat naa sila annotation or affidavit.

4. 2 PHOTOCOPY OF 2 VALID ID'S OF PRINCIPAL BUYER and SPOUSE if naa (WITH 3 SPECIMEN SIGNATURE)

Dapat Consistent signature sa ID vs Specimen Signature, e check ang expiration and kung ma klaro ba ang ID.

5. PHOTOCOPY OF TIN ID'S (BUYER AND SPOUSE)
IF walay ID pwede ang TIN verification, 2316, 1904-C.

6. ORIGINAL SPECIAL POWER OF ATTORNEY (IF NEEDED)

E ask unsa nga SPA needed

 SPA with 1 Principal –if si buyer dili ka sign or kung nay spouse dili ka sign.
 SPA with 2 principal – Kung married nya both si principal and the spouse dili ka sign personally
sa document

7. PHOTOCOPY OF 2 VALID ID'S OF THE ATTORNEY-IN-FACT (WITH 3 SPECIMEN SIGNATURE)

 If si spouse ra ang attorney in fact no need ra mangayo balik but if lahi gani ang Attorney in fact dapat mo submit
jud og ID and e inform nga ig mag BORROWERS VALIDATION na si pag-ibig both PRINCIPAL BUYER and the AIF dapat mo
attend.

PROOF OF INCOME:

NOTE: ask the buyer in a nice way kung ang income nila is declared sa BIR , ingna nga e check pud na apil ni pag-ibig
kay naa na nahitabo nga dako ang ge declare nga income and pag-abot sa BIR kay gamay ra diay.

IF PRIVATE EMPLOYED (EITHER OF THE TWO)

 UPDATED CERTIFICATE OF EMPLOYMENT WITH COMPENSATION (CERTIFIED)

 UPDATED PAYSLIP (1 MONTH) MUST BE CERTIFIED

NOTE: if naa naka indicate SYSTEM GENERATED pwede ra Electronic Signature but if System generated
pero way pirma dapat ipa Certify sa Direct superior (ipa indicate iyang position, email and contact
number same sa Payslip pag certify and sa COEC nga walay naka indicate nga system generated. Mas
better kung COEC ang e submit kay 6 months validity unlike payslip.

IF GOVERNMENT EMPLOYED (ALL NEEDED)

 UPDATED CERTIFICATE OF EMPLOYMENT WITH COMPENSATION (CERTIFIED)

 UPDATED PAYSLIP (1 MONTH) MUST BE CERTIFIED

Note: ORIGINAL ang Signature with certification of sa Direct superior (ipa indicate iyang position, email
and contact number)

IF ABROAD/OVERSEAS (EITHER OF THE THREE)

 EMPLOYMENT CONTRACT –if open contract need attachment nga Certificate of Employment
 LATEST JOB CONTRACT (WITH POEA STAMP) (SEAMAN or SEAWOMAN)
*PHOTOCOPY OF SEAMANS BOOK
*PHOTOCOPY OF LATEST EMBARKATION STAMP

IF SELF EMPLOYED with Own Business (ALL NEEDED)

 INCOME TAX RETURN (check number 18 sa ITR then divide into 12(months)
 AUDITED FINANCIAL STATEMENTS
 OFFICIAL RECEIPT OF TAX PAYMENT FROM BANK SUPPORTED
 DTI REGISTRATION
 MAYOR'S PERMIT/ BUSINESS PERMIT

Note: Year 2021 na ang pangayuon for ITR.

IF SELF EMPLOYED WORK FROM HOME/ FREELANCE (ALL NEEDED)

 CERTIFICATE OF EMPLOYMENT WITH COMPENSATION (CERTIFIED)

 LETTER OF ENGAGEMENT
 1 YEAR TRANSACTION OF REMITTANCE
 1 YEAR BANK STATEMENT
