4 Latest ITR/Cert. of Tax Witheld - BIR Form 2316 (If no tax deduction on Payslip)
5 HR / Employer valid contact details for Credit Investigation (Contact no. and email address)
6 1x1 picture (6 copies, white background, front facing and w/o accessories)
7 Marriage Contact if married (PSA or Local Civil Registrar photocopy is accepted)
8 Birth Certificate (PSA or Local Civil Registrar photocopy is accepted)
9 Photo copy of two valid id's with 3 specimen signature (SSS ID,POSTAL ID,PRC ID,VOTER'S ID,DRIVER'S LICENSE,PASSPORT)
10 Spouse Valid ID with 3 specimen signature
11 Verified BIR 1904 Form / TIN Verification Slip (with stamp verified by BIR)
12 Verified BIR 1905 Form (registration of information update/correction)
13 Signed MSVS (Membership Status Verification Slip)
14 Conformity for non-relative (if the Co-Borrower is not on the second civil degree of consanguinity or affinity)
15 Pag-Ibig Borrower's Validation
16 Pag-Ibig Health Questionnaire (if Loanable Amount is 2M)
17 Pag-Ibig Health Questionnaire with Medical Certificate (for Borrowers up to 60 years old)
18 Pag-Ibig MID number
19 Housing Loan Documents (to sign)
