In view of the issuance of Memorandum Order No. 68 dated November 6, 2020 (Re:
Updated Standard Guidelines on the strict Health Protocols to be observed in the conduct
of Licensure Examinations during the COVID-19 pandemic) by the Commission, the
following guidelines are to be strictly implemented during the conduct of the licensure
examination:

1. Report to the school/building assignment by 6:00 AM to avoid the long queue


at the gate. Present QR code and health certificate to the school guard;

2. Approach the Medical Officer and present the following for screening:
 Notice of Admission;
 Informed Consent form (original, duly accomplished and signed);
 Health Declaration forms (original, duly accomplished and signed); and
 Negative RT-PCR test laboratory result / Certificate of 14-day
Quarantine;

3. After screening, proceed immediately to your room assignment and submit


the following to the room watcher/proctor:
 Notice of Admission;
 Informed Consent form
 Health Declaration form; and
 Negative RT-PCR test laboratory result / Certificate of 14-day
Quarantine;

4. Once done, you will be assisted to your assigned seat;

5. Observe physical distancing of two (2) meters between examinees.


Examinees shall be restricted to their assigned seat;

6. Examinees are required to wear face mask (at least 3-ply surgical mask,
preferably N95 mask without valve) and face shield at all times. For identity
verification and pre-examination checking, face mask will be required to be
properly taken off temporarily to show examinee’s face and the inner part of
the face mask. Face masks are not allowed to be taken off during the
progress of the exam;

7. Face masks and face shields will only be removed when taking meals or
drinks;

8. Examinees are required to bring the following:


 70% ethyl alcohol / alcohol-based sanitizer for hand disinfection;
 Long transparent envelope;
 Long brown envelope;
 No. 2 pencils;
 Black ink ball pen (to be used ONLY in signing the Examinees Record of Attendance);
 Sharpener;
 Eraser;
 Notice of Admission;
 Optional: Tissue (for personal use).
Note: Strictly no sharing or borrowing of ball pens, pencils, erasers, etc. from
seatmate to prevent cross-contamination.

9. Regularly sanitize your hands using your own alcohol or hand sanitizer;

10. Bring your own food (for lunch and snacks) and enough water for the day;

11. During breaks, examinees will not be allowed to leave their assigned room and
will be restricted to their assigned seat except when going to the comfort room
which will be accompanied by a room watcher. Windows will be opened for
proper ventilation;

12. Proper hand hygiene must be observed particulary every after use of the comfort
room. Going to the comfort room must be one at a time and no examinee is
allowed to go out of the room unaccompanied;

13. All bags shall be placed on a designated area within the room;

14. Cellular phones must be turned off (including alarms). Once surrendered, it will
only be retrieved during breaks and after submitting the answer sheet and Test
Questionnaire (TQ) of the last subject for the day;

15. Companions are strictly not allowed;

16. Submission of answer sheet & TQ must be one at a time. When submitting your
answer sheet and TQ to the room watcher, use your own black ball pen in
signing the Examinees Record of Attendance form;

17. After submitting your answer sheet and TQ, return to your seat quietly and
refrain from talking to other examinees;

18. No talking and loitering in the hallways or in the examination room;

19. After submitting your answer sheet and TQ for the last subject of the day, quietly
collect your belongings and immediately go out of the room and leave the school
premises through the designated exit gate;
