Mastery Test MT 1.1: Hazard Identification

Group activity (Duration: 60 mins)

A facilitator is to be appointed within each group, who will coordinate the discussion. A summary
of the discussion is to be recorded in MT 1.1 Table 1 (soft copy Word document in your thumb
drive). Each group is to share your findings with the class.

Your task:
1. Review the Hinterland Airport accident investigation report.

2. Identify 3 apparent Hazards/ Threats (20 marks for each correct answer,
max. 60 marks)

3. Identify at least 1 Unsafe Situation (5 marks for each correct answer,

max. 10 marks)

4. Identify at least 1 Ultimate Consequence (5 marks for each correct answer,

max. 10 marks)

5. Identify at least 2 apparent Defences/ Barriers, regardless of whether they were

effective or not effective (5 marks for each correct answer,
max. 20 marks)

6. Annotate (list) the various Hazards/ Threats, Unsafe Situations, Ultimate

Consequence and Defences/ Barriers information (identified from steps 2 to 5) in the
appropriate columns of MT 1.1 Table 1 (soft copy Word document in your thumb
drive).

7. There is no need to correlate your identified Hazards with the Unsafe Situations,
Ultimate Consequence and Defences/ Barriers information in the other columns.

Note:
This Mastery Test is to check if you are able to recognise generic or specific hazards/ threats
information within a given safety report, and if you are also able to recognise the other safety risk
mitigation related entities, namely unsafe situations, ultimate consequences, defences/ barriers.

You are required to obtain a minimum score of 80 out of 100 in order to pass this Mastery Test.
