NARRATIVE REPORT

Webinar Title: DepEd Forms

Date: October 24, 2022
Resource Speaker: Bayani N. Escoton, LPT

In sequence to provide a trustworthy assurance mechanism for learner

information, guarantee the quality and timeliness of school reports, and decrease the
resources used for clerical and records management, the Department of Education
(DepEd) herewith specifies the standardized procedure and protocols in the
preparation, evaluation, and updating of school forms carried out at the end of each and
every school year. And that was the topic of the second webinar in the series. The
department of education is highlighting the various school forms that are required to
enforce accountability, accuracy, and reliability of data. This half-day webinar was
launched via Lark Video conferencing. After quite a relatively short prayer, the webinar
was then opened with the playing of the KCAST Hymn and the Philippine National
Anthem. After an introduction by a member of the class, the resource speaker was then
introduced.

The resource speaker subsequently introduced the 10 school forms that the
DepEd uses. Beginning with the SF 1 (School Register), which encompasses the
(Master list of Class Enrollment), moving on to the SF 2 (Learner's Daily Class
Attendance), the SF 3 (Book Issued and Returned), the SF 4 (Summary Enrollment and
Movement of Learners), the SF 5 (Report and Promotion), the SF 6 (Summary Report
of SF 5), the SF 7 (Inventory of School Personnel), the SF 8 (Learner Basic Health
Profile), the SF 9 (Learner Progress Report Card) and lastly, SF 10 (Learner's
Permanent Academic Record).

Moreover, he also identified the individuals who will be responsible for creating
the aforementioned forms, such as the class adviser or the head of school. He also
covered how each form is prepared, whether it be manually or using a LIS, and even
presented a timetable for that though. It is indeed completely accurate that creating the
forms was time-consuming and required a great deal of effort from the teachers. With
the issuance of the aforementioned policy, which supplanted manual form preparation
with the emergence of streamlined and electronic school forms made accessible for
download from the Learner Information System, the latter pressure was significantly
reduced.

The stability and accuracy of reporting school data were vastly enhanced
because the electronic school forms used in every school across the country come pre-
loaded with auto-generated data based on the most up-to-date records available.
Eventually, the webinar was brought to an end with certificates being distributed,
student impressions from their various specializations, and a final statement from a
chosen student.
