The resource speaker subsequently introduced the 10 school forms that the
DepEd uses. Beginning with the SF 1 (School Register), which encompasses the
(Master list of Class Enrollment), moving on to the SF 2 (Learner's Daily Class
Attendance), the SF 3 (Book Issued and Returned), the SF 4 (Summary Enrollment and
Movement of Learners), the SF 5 (Report and Promotion), the SF 6 (Summary Report
of SF 5), the SF 7 (Inventory of School Personnel), the SF 8 (Learner Basic Health
Profile), the SF 9 (Learner Progress Report Card) and lastly, SF 10 (Learner's
Permanent Academic Record).
Moreover, he also identified the individuals who will be responsible for creating
the aforementioned forms, such as the class adviser or the head of school. He also
covered how each form is prepared, whether it be manually or using a LIS, and even
presented a timetable for that though. It is indeed completely accurate that creating the
forms was time-consuming and required a great deal of effort from the teachers. With
the issuance of the aforementioned policy, which supplanted manual form preparation
with the emergence of streamlined and electronic school forms made accessible for
download from the Learner Information System, the latter pressure was significantly
reduced.
The stability and accuracy of reporting school data were vastly enhanced
because the electronic school forms used in every school across the country come pre-
loaded with auto-generated data based on the most up-to-date records available.
Eventually, the webinar was brought to an end with certificates being distributed,
student impressions from their various specializations, and a final statement from a
chosen student.
