is in full understanding that the maternity benefit is an SSS benefit that is merely advanced by
RESULTS MANILA INC. and my failure to submit the supporting documents which will eventually
lead to the company’s failure to collect the said amount from SSS will convert the advances
amount into a personal loan extended to me by the company.

I hereby grant full authority to “THE RESULTS COMPANIES” / “RESULTS MANILA INC.” to deduct
the full maternity cash advance amounting to PHP ___________________ from my salaries
should I fail to submit the supporting document within 30 days from my return to work or should
Social Security Services (SSS) denied/rejected my maternity benefit claim.

In the event that I get separated from the company for whatever reason, I am hereby authorizing
the company from to deduct the amount in full from my final pay.
