RTH2155 Task #3

Interpreting the 12 Lead ECG

12-Lead #1:
STEP # Interpretation
1 NSR: with bradycardia Regular rhythm. Rate: bradycardic, P waves; Upright, round,
before every QRS. PRI: 0.2s, QRS: .12s
2 normal axis deviation
3 Regular QRS interval & morphology
4 J point Identified, Apparent ST elevation in leads ll, lll, and aVF
5
6 It appears that the Inferior side of the heart has had a STEMI
7 The RCA or LCx have been occluded

12-Lead #2:
STEP # RESULTS
1 Regular rhythm, Rate: Bradycardic (57 BPM), P waves: upright, round, before each QRS.
PRI: 0.2s, QRS: .08s
2 Normal Axis Deviation
3 QRS interval: <0.12s, regular morphology in lead V1
4 ST elevation in leads V3, V4, V5, V6
5 ST depression in leads ll, lll, and aVF
6 It appears that a STEMI has occurred in anterior lateral portion of the heart
7 The diagonal branch of the LAD or LCx have been occluded.

12-Lead #3:
STEP # RESULTS
1 Rhythm: Regular, Rate: 94 BPM, P waves upright, round, before every QRS PRI: 0.2s,
QRS: 0.16s
2 Left axis deviation
3 Right Bundle Branch Block
4 No ST elevation
5 N/A
6 There is no Elevation, only depressions
7 NSTEMI
12-Lead #4:
STEP # RESULTS
1 Idioventricular rhythm, Rhythm: Irregular, Rate: 38 BPM, P waves none,
2 Normal axis deviation
3 QRS: 0.12s
4
5 ST elevation in leads ll, lll, and aVF without reciprocal change
6 STEMI in inferior part of the heart
7 LCx is likely occluded due to ST depression in aVL, but RCA is still possible

12-Lead #5:
STEP # RESULTS
1 Rhythm: Irregular, Rate 60 BPM, P waves: N/A, QRS: 0.08s
2 Axis is deviated to the Left side
3 QRS interval: 0.12s
4 No ST depression or elevation
5 No ST Elevation
6 N/A
7 N/A

12-Lead #6:
STEP # RESULTS
1 First degree heart block. Rhythm: Regular, Rate: 65 bpm, P waves: upright, regular,
before QRS, PRI- 0.22s
2 Left axis deviation
3 QRS is normal
4 ST elevation in leads V1 – 6 , lead , aVL
5 No reciprocal changes
6 Anterioseptal MI
7 LAD or / and LCx
