A RESOLUTION TO OPEN SLPA PASSBOOK SAVINGS ACCOUNT AT RURAL

BANK OF KIAMBA, KIAMBA, SARANGANI PROVINCE

Whereas, the Pinol Kauyagan – SLPA, President, Treasurer and Identified SLPA
officers go to the nearest legitimate rural bank operating within or adjacent to
the City to open an SLPA passbook savings account.

Whereas, the membership fee which includes pre-operating expenses shall be used
as a deposit fee for the opening of an account of the SLPA. The amount
needed for the savings account shall be equally paid by each member of the
SLPA.

Whereas, the signatories shall be the President, Treasurer, and Elected SLPA
officer who may jointly withdraw grants.

Whereas, the members of the association have determined it to be in the best

interest of the association to open an account with Enterprise Bank,
Magsaysay Avenue, General Santos City, so be it;

Resolved, that the association do open an account with Rural Bank of Kiamba with
a registered address at Municipality of Kiamba, Sarangani Province;

Further Resolved, that the following be and are hereby authorized to sign, execute,
and submit all documents in connection with the opening of the account with
the above-said branch of Rural Bank of Kiamba including verification of
signatures of authorized signatories as may be required by the Enterprise
Bank.

Further Resolved, that the above representative is hereby authorized to transact

business, including but not limited to, the maintenance of savings, checking,
and other accounts, as well as borrowing by the association and other
 account-related transactions deemed necessary for the representative to
perform in behalf of the association.

October 20, 2022.
