LETTER OF PARENTAL CONSENT

To all whom this may concern,

I, KURT GARCIA GATUS, the father of KURT GARCIA GATUS, hereby give my consent for
his clinical internship to be done within the hospital setting outside of the Adventist University
of the Philippines. We have discussed the possibility of this happening; we understand and take
full responsibility for the risks this may pose. This is because we feel that for him to be ready for
the actual job, it is best to do his clinical internship at the hospital.

Furthermore, we are absolving the Adventist University of the Philippines of any liability in the
event that our son contracts COVID-19.
