Living condition: Three family members live together in a HDB 2-room flat.
Maisie, aged 41, is at the polyclinic for prenatal check-up today.
She is currently 12 weeks pregnant, has a history of 1 normal vaginal delivery of a boy at 36 weeks of
gestation in 2016 and 2 spontaneous abortions at 13 weeks of gestation in 2018 and 8 weeks of
gestation in 2020.
You are the staff nurse attending to Maisie.

Gravida: 4, Para: 1 (G4P1)


Expected Date of Delivery (EDD): 9th January 2023
Height: 160 cm
Weight before current pregnancy: 75kg
Current weight: 80kg

Medical / surgical history: Femoral herniorrhaphy in 2021


Family history: Maisie’s father and mother have Type 2 Diabetes Mellitus
Blood Pressure: 130 / 78 mmHg
Pain score: 2 / 10 for lower backache
Urine dipstick test results: to collect

Maisie shares with you that she worries about:


• the growing fetus and pregnancy complications.
• antenatal screening and cost for pregnancy.
• the arrival of her newborn due to the lack of family support.

Your assessment / conversation with Maisie shows:


• she is inactive and rests on the sofa most of the time.
• she has no energy to cook for the family and eats out most of the time.
• she drinks 2 cans of energy drink like Red Bull, 2 cups of bubble milk tea with 100% sugar and 2
cups of coffee a day to stay awake.
• she has craving for sugary treats and desserts like cupcakes.
• she likes to eat desserts e.g., ice-cream and chendol up to 4 times a week.
• she dislikes taking medications and often skips her multi-vitamin pills prescribed by doctor as she
wants to avoid the side effects.

Your assessment / conversation about her husband, Malcom reveals:


• he has been working the graveyard shift for 10 to 12 hours a day.
• he works long hours, lacks exercise, eats unhealthy late-night meals, and sometimes skips meals.
• he is stress and fatigue from the prolonged working hours and finds it hard to make ends meet.
• he smokes at home often and smokes about 25 cigarettes per day to stay alert and relieve stress.
• he loves to meet his friends for drinking sessions during off days and often drinks up to 5 cans of
beer.

Your assessment / conversation about her 6 years old son, Mason shows:
• he does not eat fruits and vegetables and consumes fast-food three times a week or more.
• he does not like to exercise and has a body mass index (BMI) of 20.
• he grows up watching television regularly.
• the family resorts to online videos and games to keep him occupied at home.
• he sleeps about 7-8 hours a night.
• he skips school most of the times due to asthma and allergic rhinitis symptoms.
• he has difficulty paying attention in school.
Maisie is keen to adopt a healthier lifestyle for herself and family but requires more information and
guidance.

ICA Individual Assignment Framework


1. INTRODUCTION

o Clearly identify and state ONE actual / potential health issue from the case study to discuss.

1.2 [Clinical Significance]


o Discuss the background information about the identified health issue – state the clinical
significance and the importance of health promotion for the identified health issue.

1.3 [Scope]
o Outline the content / scope to be discussed in your essay.

Helping Notes:
Essay has three parts:
- Introduction
- Body
- Conclusion

The introduction has three main purposes:


- It provides background information
- It interests your reader in your topic
- It presents your thesis statement.

A thesis statement is a short statement, usually one sentence, that summarizes the main point
or claim of an essay, research paper, etc., and is developed, supported, and explained in the text
by means of examples and evidence.

2. BODY

To focus and discuss on the identified health issue in depth and professionally.

2.1 [Definition, Risk Factors, Clinical Manifestations, Complications]


o State the definition, risk factors, signs & symptoms, and complications of the identified health
issue.

2.2 [Health Promotion Strategies]


o Discuss practical health promotion and disease prevention strategies to tackle the identified
health issue. To elaborate and provide examples where appropriate.

2.3 [Community Resources]


o Identify and introduce community resources (e.g., centres, facilities, programmes, classes,
workshops, support groups, services, initiatives, campaigns, schemes, packages, guidelines,
smart phone applications) to help promote / maintain health.
2.4 [Role of the Nurse]
o Critically discuss and cite examples on how the nurse can participate, facilitate, promote, and
maintain health of the client, family, and community.

Helping Notes:
Each paragraph has 3 parts:
- Topic sentence
- Supporting details (e.g., facts and examples, that back up the claim made in that sentence)
- Concluding sentence

A topic sentence is a sentence that captures the meaning of the entire paragraph or group of
sentences. It tells what the passage is mainly about.

3. CONCLUSION
o Restate the thesis and summarize the essay’s body,

The conclusion has three main purposes:
- re-emphasises the thesis statement
- draws the essay to a close
- summarises the key points discussed

Helping Notes:
- It’s NOT about introducing new ideas but summing up the writing.
- A conclusion provides closure and drives the main points of your essay one last time.
- It’s the chance to impress and give readers an understanding of why your paper matters.

ICA Individual Assignment Guidelines

 Refer to ICA Individual Assignment Instructions and ICA Individual Assignment Rubric.
Follow the ICA Individual Assignment Framework, however it is not exhaustive.

 Provide evidence-based discussion and resources in your essay.


Utilise and indicate a minimum reference of FIVE (5) scholarly literatures (books, journals, and
reliable e-resources).

 Provide in-text citations and an end reference list.


Absence / incorrect / incomplete in-text citations and end reference list will be PENALISED.

 Present the individual assignment in accordance with the:

 The ICA Individual Assignment MUST be an INDIVIDUAL EFFORT.

 Stay within the word limit given (1000 words, +/- 10%).
Content that is over the 10% limit will NOT be graded.

o Keep direct quotations to a minimum. As a guide NOT > 5% of your entire assignment.

o Use a minimum of double line spacing.

o Use font Times New Roman, size 12.

o Use 1-inch margins on every side of the page.

o Indicate your name, admin number and tutorial group at the footer of the document.

o Instructions on the submission of Individual Assignment (soft copy) via Brightspace:

o Complete the online student declaration on Brightspace as part of the requirement for


assignment submission.

o Submit individual assignment in Microsoft Word document / format.

o Rename and attach the document for a SINGLE submission:

Document should include:

 Content of actual individual assignment (introduction, body & conclusion)

 Total word count of assignment. Word count should include all words in the main
body of the assignment.

 Abstract (not required), appendices (not required) and references are NOT included
in the word count.

 End Reference List

 Rename file as: Name_admission no._Written Assignment

 Students are required to submit their assignment to Turnitin in BS and to ensure that the
similarity index (SI) is kept ≤20%.

 If the similarity index is 20% or more, review the originality report and scrutinise the areas
highlighted, rephrase, rewrite in your own words, and resubmit the individual assignment.
 Take note that the latest submission will override the previous submissions.

 SI report will be generated after 24 hours for the fourth and subsequent submissions.

 In view of the similarity index checking, possible rewriting and resubmission, and similarity index re-
checking, students are strongly advised to submit the individual assignment via Brightspace EARLY –
at least 3 to 7 days before the submission deadline.

 Plagiarism is strictly forbidden.


Students with similarity index 20% or more, will be reported and PENALISED for plagiarism.

 Students are reminded that


(a) plagiarism (i.e., copying another person’s work without proper referencing) and
(b) abetment to plagiarism (i.e., sharing of your work with others) are strictly prohibited and
subjected to disciplinary action, if / when committed.

Any student caught plagiarising the work of some other student(s) or who deliberately use an
author’s ideas or words without the accompanying reference(s) will be reported and may be
subjected to DISCIPLINARY ACTION.

 Students must ensure correct and successful submission that meets the ICA instructions and
requirements. Students must keep copies of their original work, originality report, and the
acknowledgement email / receipt of successful submission of assignment as proofs.

 LATE / NIL submission of individual assignment will be PENALISED and students risk failing the
assignment component / module.

Where a student submits a late assignment (after the due date), and has NOT been granted an
extension (with valid reason / evidence), the assigned work will be penalised as follows:

 5% of the assignment base mark will be deducted for late submission for each calendar day up
to a maximum of 3 calendar days or 15% deduction.

Example: base mark is 50, awarded mark is 30. Hence, 5% of 50 will be deducted for one day
late submission.

 Assignments submitted after 3 calendar days following the due date will be reviewed but NOT
graded and marks will not be awarded.
