Living condition: Three family members live together in a HDB 2-room flat.
Maisie, aged 41, is at the polyclinic for prenatal check-up today.
She is currently 12 weeks pregnant, has a history of 1 normal vaginal delivery of a boy at 36 weeks of
gestation in 2016 and 2 spontaneous abortions at 13 weeks of gestation in 2018 and 8 weeks of
gestation in 2020.
You are the staff nurse attending to Maisie.
Your assessment / conversation about her 6 years old son, Mason shows:
• he does not eat fruits and vegetables and consumes fast-food three times a week or more.
• he does not like to exercise and has a body mass index (BMI) of 20.
• he grows up watching television regularly.
• the family resorts to online videos and games to keep him occupied at home.
• he sleeps about 7-8 hours a night.
• he skips school most of the times due to asthma and allergic rhinitis symptoms.
• he has difficulty paying attention in school.
Maisie is keen to adopt a healthier lifestyle for herself and family but requires more information and
guidance.

Essay has three parts:
- Introduction
- Body
- Conclusion
A thesis statement is a short statement, usually one sentence, that summarizes the main point
or claim of an essay, research paper, etc., and is developed, supported, and explained in the text
by means of examples and evidence.
2. BODY
To focus and discuss on the identified health issue in depth and professionally.
Helping Notes:
Each paragraph has 3 parts:
- Topic sentence
- Supporting details (e.g., facts and examples, that back up the claim made in that sentence)
- Concluding sentence
A topic sentence is a sentence that captures the meaning of the entire paragraph or group of
sentences. It tells what the passage is mainly about.
3. CONCLUSION
o Restate the thesis and summarize the essay’s body,

The conclusion has three main purposes:
- re-emphasises the thesis statement
- draws the essay to a close
- summarises the key points discussed
Helping Notes:
- It’s NOT about introducing new ideas but summing up the writing.
- A conclusion provides closure and drives the main points of your essay one last time.
- It’s the chance to impress and give readers an understanding of why your paper matters.

Follow the ICA Individual Assignment Framework, however it is not exhaustive.
Stay within the word limit given (1000 words, +/- 10%).
Content that is over the 10% limit will NOT be graded.
o Keep direct quotations to a minimum. As a guide NOT > 5% of your entire assignment.
o Indicate your name, admin number and tutorial group at the footer of the document.
Total word count of assignment. Word count should include all words in the main
body of the assignment.
Abstract (not required), appendices (not required) and references are NOT included
in the word count.
Students are required to submit their assignment to Turnitin in BS and to ensure that the
similarity index (SI) is kept ≤20%.
If the similarity index is 20% or more, review the originality report and scrutinise the areas
highlighted, rephrase, rewrite in your own words, and resubmit the individual assignment.
Take note that the latest submission will override the previous submissions.
SI report will be generated after 24 hours for the fourth and subsequent submissions.
In view of the similarity index checking, possible rewriting and resubmission, and similarity index re-
checking, students are strongly advised to submit the individual assignment via Brightspace EARLY –
at least 3 to 7 days before the submission deadline.
Any student caught plagiarising the work of some other student(s) or who deliberately use an
author’s ideas or words without the accompanying reference(s) will be reported and may be
subjected to DISCIPLINARY ACTION.
Students must ensure correct and successful submission that meets the ICA instructions and
requirements. Students must keep copies of their original work, originality report, and the
acknowledgement email / receipt of successful submission of assignment as proofs.
LATE / NIL submission of individual assignment will be PENALISED and students risk failing the
assignment component / module.
Where a student submits a late assignment (after the due date), and has NOT been granted an
extension (with valid reason / evidence), the assigned work will be penalised as follows:
5% of the assignment base mark will be deducted for late submission for each calendar day up
to a maximum of 3 calendar days or 15% deduction.
Example: base mark is 50, awarded mark is 30. Hence, 5% of 50 will be deducted for one day
late submission.
Assignments submitted after 3 calendar days following the due date will be reviewed but NOT
graded and marks will not be awarded.
