Common Requirements
1. Sworn Registration Statement using either.
2. Certified True Copy of Transfer Certificate of Title (TCT) or Original Certificate of Title by
The Register of Deeds
2.a. Affidavit of the owner that the property is free form liens and encumbrances (Form 1120)
3. Duly audited balance sheet (certified copy of the original) issued and signed by the authorized
Official of the applicant and under the latter’s authority.
5. Copy of any circular, prospectus, brochure, advertisement, or communication used/to be used for
Public offering of subject project and for circulation upon approval by the board.
6. Sample copy of Contract to Sell to be used in the public offering of lots, units or lots and units
7. Affidavit, re: inclusion the provision of P.D. 957 of the contract to sell (HLRB Form No. R-1)
12. Certification from the Local Water District / Franchise Holder that they will serve the project

c. Project Study
REQUIREMENTS FOR LICENSE TO SELL:
A. Program of Development (bar chart with S-curve, Gant Chart / PERT-CPM, ETC.) signed and sealed by
Licensed engineer or architecture indicating work activities, duration/time frame of activities.
B. Affidavit of undertaking to submit proof of Title issuance for all saleable lots & submit certified
Xerox copy of titles for common areas (i.e. road lots, open space, community facility, etc.)
C. In case the approval is with housing component: Building permit, cost estimates and approval building
plans.
Note:
For subdivision: Required a minimum development prior to the issuance of CR & LS – Land clearing &
grubbing, road tracing, earthworks, sub-base, and base preparation activities, and entrance gate if
included in the advertisement or development plan.
For Condominium: civil and structural works of foundation to include excavation, pile driving
concreting, steel reinforcement and form works
Subdivision Plans. Topographic Map, Engineering Plans (alignments, elevation, typical cross sections and
details of system roads, drainage and sewerage disposal, water and power distribution and street
lighting), Tree Planting Scheme, Cost Estimate on Land Development.
VICINITY MAP
SITE DEVELOPMENT PLAN – showing the proposal lay-out of the streets. Lots, parks and
playground and other features with percentage and area of land allocation: vicinity map: Lot
numbers, lines and areas and block numbers (signed and sealed by the architect/ Civil
Engr/Geodetic Engr./ EnP)
TOPOGRAPHIC PLAN– to include boundary lines; street easement, width and elevation of road
right of way; utilities and within and adjacent subdivision project; ground elevation of
subdivision; water course proposed public improvements (signed and sealed by Geodetic Engr.)
SITE GRADING PLAN – with finished contour lines superimposed on the existing ground limits of
earthwork embankment slopes (signed and sealed by Civil Engr.)
DRAINAGE NETWROK PLAN – profile showing hydraulic gradients and properties and details of
storm drainage and other miscellaneous structures such as manholes, catch basins, inlets (curb,
gutter, and drop), culverts and channel lining. (Signed and sealed by Civil Engr.)
WATER SYSTEM LAY-OUT PLAN – reflecting fire hydrant (signed and sealed by Civil Engr.); should
a pump motor has horse (hp) rating 50 hp or more- signed by Mechanical Engr.
TREE PLANTING SCHEME- indicate plant/tree species and other natural/manmade landscaping
features (signed and sealed by Civil Engr./ Landscape Architect)
