SYSTEM PERFORMANCE CERTIFICATE

This certification is issued to Polymedic Medical Plaza Hospital – Cagayan De Oro City for the
GE Vivid S70 with SN: 200542S70 including system probe and accessories, currently installed at
Heart Station.

Tests are done in accordance to the test inspection limits and/or expected results derived from
the equipment’s service manuals, and that the equipment complies with all test checkpoints as
laid out in the equipment installation form.
