SUBJECT: SOCIAL OUTREACH PROGRAMME REPORT

This report should give a brief account of each student’s experience of the Social Outreach Programme.
The report should be done in APA format.

The report should contain the following:

1. Title Page
a. Student’s Name
b. School, group and ID#
c. Name of the Coordinator
d. Title of the Project (Social Outreach Programme Project)
e. Name and address of organization in which the project was done
f. Name of supervisor or contact person at the organization
g. Date of submission of the report

2. The Body of the Report

a. Explain the duties performed
b. Show how the organization in which you worked benefited from your participation in
the programme
c. State whether you would recommend the institution in which you worked for future
S.O.P. Project. Give reasons.
d. How has participation in the programme changed your perception of volunteerism and
how has it impacted your life
e. What are your recommendations for the improvement of the Social Outreach
Programme. (We value your recommendations highly.)

3. Appendix:
a. Programme card, completed and stamped by the organization
b. Evidence of student involved in voluntary activities

Reports must be submitted on or before April 30, 2022. NO LATE REPORT WILL BE ACCEPTED. Please
upload all reports to canvas. Students who participated using the organizational approach are required
to take a picture of the outreach card and upload it with their reports, however, the hard copy must be
submitted to Mrs. Nadine Owen-Russell in the Student Services Division (glass door opposite Students
Affairs).
