1. Purpose:- As per the statues, it is mandatory that a bridging document is


prepared for the smooth and safe operations at an installation where two or
more independent activities are performed or conducted by different
agencies. This document tries to identify proposed interactions and mode of
management. This document also covers identified specific risks that may
arise as a result of the proposed or likely interactions and management
controls to mitigate or eliminate the risks. The document also specifies the
roles and responsibilities of each operator.Document shall govern the various
emergency response and operational safety procedures to be followed while
performing their job inside the installationto ensure compliance with ONGC’s
safety and environmental management system requirements.

2. Brief on Contractor:-M/s HAL Offshore, Mumbai is awarded the contract to


maintain a Gas De-hydration Unit at the installation to de-hydrate the
processed gas received before it is despatched into the GAIL pipelines. The
contract is valid for period of five years and during the period the contractor
will operate & maintain the associated equipment.

3. Identified Interactions:-The identified interactions between the operators of


M/s HAL offshore and ONGC are,
a) Maintaining and monitoring of operational Parameters as per the
contract
b) Documents to be maintained as per statutes requirement
c) Documents to be maintained as per ONGC requirement
d) Internal and External Safety audits
e) Training of operators and supervisors as per MVTR 1966
f) Medical examination of crew members as per statute.

g) Site specific Emergency Response plans and defined roles and


responsibilities.
h) Maintaining & using of Statutory requirements like PPE, SOP ,
displays , tools and shackles
i) Implementation of Codes of safe practices like Permit to work, tool
box talks, JSA, LOTO etc
j) Reporting of incidents viz. near misses, minor, major & fatal

4. Mode of Management:-
I. Maintaining and monitoring of operational Parameters:

 Daily: M/s HAL Supervisor to submit a Daily Progress report which


includes critical operational parameters and deviations from normal (if
any) with remarks to Residential Engineer/ Shift In-charge ONGC.
 Maintenance Works: M/s HAL supervisor to get clearance from
Installation Manager & ATSM for any Maintenance works with respect
to GDU.
 Emergency Scenarios: During Emergency scenarios, ONGC initiates
its Emergency Response plan and M/s HAL offshore to follow it and
M/s HAL to follow the instructions of the Onsite Commander,ONGC. In
case of an emergency situation that call for activation of ERP with
respect to M/s HAL offshore , the contractor will follow the same
procedure of sounding the alarms and following the instruction of onsite
commander. Under the both scenarios it is pertinent that power
disconnection and use of fire fighting resources should be done for the
protection of whole installation.

 Work permit records
 Safety briefing records
 All statutory compliances and approvals

III. Documents to be maintained as per ONGC requirement:

 All process parameters and other details in accordance with


contract

IV. Internal and External Safety audits:

 M/s HAL Offshore to comply with all the necessary records to be


maintained and shall cooperate with the Internal & External Audit
teams in consultation with the ONGC Residential Engineer/ Shift
Incharge

V. Training of operators and supervisors as per MVTR 1966

 All the crew members of M/s HAL offshore have to undergo Mines
Vocational Training as per MVT rules 1966. The Training shall be
provided to the contractors at General Vocational Training Centre,
ONGC, Neravy complex, Karaikal.

VI. Medical examination of crew members as per statute:

 Initial Medical Examination and there after periodic Medical


Examination is mandatory for the members working in Mines for six
months or above. Hence, All the crew members of M/s HAL
offshore has to undergo Initial Medical Examination as advised by
ONGC Medical Officer and copy of the Medical Examination results
along with Form-O to be submitted to ONGC.A copy to be
maintained at site

VII. Site specific Emergency Response plans and defined roles and
responsibilities:

 M/s HAL offshore to prepare site specific Emergency Response


Plans with detailed description of roles and responsibility of all the
operators and communication with ONGC.To be prepared in
consultation with RE/ IM. A copy to be maintained at site.

VIII. Maintaining & using of Statutory requirements like PPE, SOP ,


Safety display boards , tools and shackles:

 Job specific PPE shall always be worn by operators of M/s HAL


offshore. SOP of major operations to be displayed in field. Various
safety Boards& MSDS to create awareness on safety to be
displayed. Proper tools to be selected while performing the jobs.

IX. Implementation of Codes of safe practices like Permit to work,


tool box talks, JSA, LOTO etc:

 All Non routine works are to be carried out only with a valid work
permit. ONGC will issue the required work permit. Requisite tool box
talks,JSA and where ever required necessary lockout tag out (LOTO)
need to be taken. All records in this regard need to be maintained at
site. Cross linking of permits and LOTO need to be evidenced.

5. Identified specific risks:

The identified risks in the installations are as follows,


I. Oil spill ( In ONGC pipelines & storage tanks)
II. Gas leakage (In ONGC pipelines & GDU contract pipelines)
III. Fire (In ONGC areas and GDU contract areas)
IV. Snake bite ( Both ONGC & Contractual persons)
V. Natural calamities ( Effects both ONGC & GDU contractor)
VI. Bomb threats ( Effects both ONGC & GDU contractor)
VII. Terrorist acts ( Effects both ONGC & GDU contractor)
VIII. Electrical emergency (In ONGC MCC/PCC or Contractor MCC/PCC or
both)
IX. Medical evacuation ( Both ONGC & Contractual persons)
X. MEG leakage ( From GDU pipelines or storage tanks)
XI. Condensate leakage ( From GDU pipelines/tanks or ONGC lines or
tanks)

6. Emergency response plans

Site specific Emergency response plans with respect to various identified


scenarios with respect to ONGC is already available at site. The contractor is
required to make necessary ERPs with respect to all the scenarios that affect
them taking into consideration the available ONGC ERPs. The roles and
responsibilities of each of the operators/crew need to be explicitly mentioned.
The communication protocol and the hierarchal command and control should
be covered. The assembly points will be as designated by ONGC and the
onsite commander will be from ONGC. The onsite commander will be
assisted by the contractor supervisor and his team.

Dos and Don’ts pertaining to various activities will be displayed to avoid


confusions.

7. Roles and Responsibilities


ONGC:-
i. Shall provide all supports and guidance to the contractors as required
by them for smooth operation inside the installation as covered in the
contract.
ii. ONGC representative will be the onsite commander in the event of any
exigencies or emergencies.
iii. ONGC will provide the contact numbers of the key officers pertaining to
the installation.
M/s HAL Offshore:-
i. Will identify and notify the operators shift wise to the ONGC shift in
charge. The list will be displayed appropriately in the control room.
ii. Will identify the supervisor at site with whom the ONGC representative
could communicate.
iii. Shall prepare site specific ERPs and SOPs in consultation with ONGC
representatives and display same with due approval.
iv. Ensure that the manpower deployed are competent with appropriate
qualifications and experience as stipulated in the OMR 84 and other
relevant statues.
v. Shall follow, obtain and adopt all statutory compliances.
vi. Shall report to the identified ONGC representative at the site.
vii. Shall cooperate in the event of internal and external audits by auditing
agencies.
viii. Shall comply to the observations and violations as brought out by the
auditing agencies.
ix. Shall provide the contact numbers of the key persons to ONGC.
