EMPLOYMENT EQUIPMENT BOND FORM

This form is the agreement between Rizal Commercial Banking Corporation and

_________________ in accordance with the company rules and guidelines for Training and Pre

Employment Equipment Endorsement protocols.

Rizal Commercial Banking Corporation will be endorsing the employee the following

training equipments listed as follows;

● 1 HP EliteBook 840
● 1 Vivo X80 Phone
● 1 Huawei Internet Receiver
● 1 2TB Hard Drive

The said employee agrees to deposit P5990.00 (FIVE THOUSAND NINE HUNDRED
NINETY PESOS) for the Equipment Security Deposit . In instance, the said employee will
receive the said Training Equipments for training with or without the major security
requirements.

On the contrary, if the employee will be able to secure the MAJOR SECURITY
REQUIREMENTS (NBI clearance, Police Clearance and Barangay Clearance) within 15
working days, the employer agrees to refund the full amount of the Equipment Bond
requirement. The employee also agrees that if he/she fails to comply with this requirement the
company has the right to terminate the said job offer given to the employee.
