JOB DESCRIPTION

Position Title: Youth Worker – Transitional Living Program


Reports to: TLP Case Manager – Site Coordinator
Position Status: Non-exempt

Distinguishing Features of Work


This position is responsible for assisting in implementation and maintenance of a nurturing
environmnet in which client needs can be satisified. Youth workers support the ongoing
personal development of teenagers as they learn to become responsible young adults.

Essential Duties and Responsibilities


 Plan activities and events that support the objectives of the agency, and its service plan
agreement and also age appropriate
 Assessment of the needs and interests of the group, development of programs based on
assessment results and implement programs and necessary
 Mentor clients in relation to individual development goals, provide guidance and advise
regarding job decisions, school activities, and relationship building
 Be prepared to serve as a first hand counselor in crisis situations
 Assist in facilitating the educational goals of each participant by assisting with the
American School Program and homework assignments
 Assists the participants in developing their Life Skills by assisting with meal planning,
grocery shopping, meal preparation and budgeting
 Teach life principles through the established value-based curriculum of the youth
program
 Maintain high moral and ethical responsibilities to the participants, and maintains a
professional stance with referring and collateral organizations through actions and
speaking in order to maintain positive influence and continuous instill life values to our
clients
 Maintain and complete accurate daily logs and reports on each participants with detailed
notation of progress, achievements, deficiencies, and unusual incidents
 Supervise the routine cleaning and maintenance of participant’s apartments
 Maintain order and safety of facility and grounds by conducting hourly apartment checks
 Participates in quality improvement process by maintaining AP5 file organizations and
attendance at required meetings
 Build trusting relationships with participants and effectively interact with clients from
diverse cultures while maintaining appropriate relationship boundaries
 Attend staff meetings and training as required
Professionalism and Work Conduct
 Demonstrate integrity and high ethical standards, and display a positive image of the agency
 Act in a professional manner at all times and maintain appropriate boundaries with staff
 Demonstrate accountability for results and keeps commitments to others
 Report to work, meetings, training, and job related activities prepared as scheduled and ready to
participate as an active resource in meetings and on committees
 Respond to clients, agency staff, service providers and other stakeholders in a timely manner
and to ensure follow up that adequately addresses the need
 Perform other related tasks as required

Education and Licensing Requirements


 High School Diploma or GED
 Access to a car and valid Illinois driver’s license required
 At least 23years of age

Knowledge, Qualifications, and Skills


 Excellent communications skills
 Must possess the knowledge, skills and mental development of a mature adult
 Efficient ability to prioritize and respond quickly to document requests

Physical Demands and Work Environment

May occasionally be required to lift up to 50lbs.
