    DETAILED JUSTIFICATION

    This is to certify that, Reyes, Karl Marxcuz Rivera, a Data Protection Officer of De
    Los Santos Medical Center with office address at 201 E. Rodriguez Sr. Avenue,
    Quezon City is assigned at Quality Management and Compliance Division and
    directly catering/catered/ exposed to persons who are classified as either suspect,
    probable or confirmed COVID 19 cases in the performance of his functions and
    duties.
    His detailed COVID related functions/work assignments are:

    Close coordination with multiple internal and external stakeholders as regards


    contract draft, review and sign-off during the COVID-19 surge.
    Close coordination with doctors, patients/patients’ relatives and other allied health
    professionals regarding data subjects’ requests, internal data access request forms,
    data privacy requests and other patient related concerns on contact tracing, and
    MPH vaccination program.
    Closed contact to doctors/employees and patients who are classified as suspected
    and confirmed COVID-19 positive.
    Close-in setting with poor ventilated room with covid positive persons.

    This certification is being issued upon the request of Reyes, Karl Marxcuz R., for
    COVID 19 compensation claim purposes only.
