Identifying and Discussion of Dementia

Marc: Dementia is not a specific disease but is rather a general term for the impaired
ability to remember, think, or make decisions that interferes with doing everyday
activities. Alzheimer’s disease is the most common type of dementia. Though
dementia mostly affects older adults, it is not a part of normal aging.

Signs and symptoms:

 Memory
 Attention
 Communication
 Reasoning, judgment, and problem solving
 Visual perception beyond typical age-related changes in vision

Signs that may point to dementia include:

 Getting lost in a familiar neighborhood


 Using unusual words to refer to familiar objects
 Forgetting the name of a close family member or friend
 Forgetting old memories
 Not being able to complete tasks independently

Nursing Intervention for Dementia

Bayle: *knock, walks into the patient room and introduce self*

Patient: *patient looks confuse* What day is today? What time is it? Where am I?
What am I doing here?

Bayle: *speaks slowly and clearly* Hello ma’am/sir, today is Thursday of December
15, 2022, and the time is already 12 o’clock in the afternoon, you are in your room
ma’am/sir at your house. I will be here to guide and care for you ma’am/sir as you
regain your consciousness about your surroundings. Later my partner will be here to
give your medicine.

Administering of Medication

Aidam: *knock, walks into the patient room and introduce self*

Aidam: So I will be administering your medication maam/sir which is the Donepezil.


This medicine will help you to treat some of the symptoms of your dementia like
confusion.

Health Teaching Plan

Andres: Agitated or violent behavior can be caused by personality changes brought


on by dementia. Mild-mannered people can become aggressive or lose inhibitions
because of changes in their brain. Aggressive behavior can also be caused by physical
discomfort or unexpressed emotions, such as frustration, fear or anger.

Prevent aggression by following these tips:

 Maintain a daily routine – keep a regular schedule for sleeping, eating and other
normal activities. Routines help soothe patients with dementia.

 Involve the patient in exercise to keep them energized during the day, which will
help them sleep at night.
