CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENTS:

This Contract of Sale is entered into by and between:

_____________________, Filipino, of legal age, single and with residence at

_________________________________,
Philippines, hereinafter called the LESSOR,

________________, Filipino, of legal age, single and with residence at

____________________, Philippines, hereinafter called the LESSEE,

1. That the Lessor is the owner of a house and lot situated at ___________________ as
evidenced by TCT No. ______;

2. That the Lessor hereby agrees to lease a portion of the above-mentioned property to
the Lessee for a period of ____ year(s) starting ______________;

3. That the Lessee agrees to pay a monthly rental of P ________________ payable in

advance within the first five (5) days of the month;

4. That the Lessee agrees to pay one (1) month advance rental and two (2) months
deposit;

5. That the premises leased have been received by the Lessee in good and habitable
condition;

6. That all ordinary repairs within the premises that arise in the daily use of the
facilities therein shall be for the sole account and expense of the Lessee, without right
of reimbursement;

7. That the Lessee is expressly prohibited to assign or sublet the whole or a portion of
the leased premises to anyone, including the acceptance of boarders or bedspacers,
without the express consent of the Lessor in writing;

8. That all charges for water, light, gas, telephone used within the premises shall be at
the sole account of the Lessee;

9. That the Lessee shall notify the Lessor at least thirty (30) days in advance should the
Lessee decide to abandon the leased premises; and

10. That violation of any of the above terms and conditions will produce ipso facto the
rescission of this contract of lease.
