    (d) List of Members in good standing

    (e) Copy of Constitution and By-Laws

    (f) Certificate/Endorsement from Competent Authority

    (g) BYC Accreditation Certificate

    (prepare soft copy for YORP Registration)

    Certificate/Endorsement for
    COMMUNITY-BASED ORGANIZATION

    (a) If organization has a physical office, Certification of Existence of Office from the barangay where
    the office is located

    (b) If organization does not have a physical office, Barangay Certification of Residence of the
    President

    (c) Resolution of endorsement coming from the Sangguniang Kabataan

    Certificate/Endorsement for
    SCHOOL-BASED ORGANIZATION

    Certificate of Registration or Recognition from a competent school authority supervising student affairs

    Certificate/Endorsement for
    FAITH-BASED ORGANIZATION

    Certificate of Registration or Recognition from any head/pastor of congregation or parish priest

    Certificate/Endorsement for
    CHAPTERS OF MULTI-LEVEL ORGANIZATION

    Certificate of Registration or Recognition from the president governing at the highest organizational
    level

    Certificate/Endorsement for
    CONSORTIUMS

    Certificate of Registration or Recognition issued by the secretariat/board
