PARENT’S CONSENT

I, ________________ of legal age, a parent/guardian of Mr/Ms. _____________,

hereby permit him/her to undergo a practice regarding Drills and Ceremonies every
Sunday afternoon after the ROTC training in preparation for the anticipated Regional
Annual Administrative and Tactical Inspection at Sorsogon State University Main
Campus. And I hereby further agree to shoulder all personal and incidental expenses
that may be incurred by the student in connection with the practice on Drills and
Ceremonies.
