Biometric Identifier Collection Authorization Form

Motorola Solutions will capture and use your Biometric data, a fingerprint scan, to capture your
time reporting for your hours worked. Your data will not be disclosed by Motorola Solutions,
except to a Biometric Identifier Collection vendor or licensor, without your consent unless the
disclosure is required by law or by valid legal subpoena. If retained, your fingerprint data will be
permanently deleted from Motorola’s systems within 6 months after the purpose of obtaining
such biometric data has been satisfied or as long as required by law.

By acknowledging electronically the receipt of this form, you consent to Motorola Solutions
collection, use, and storage of your fingerprints for the above stated purpose.
