In keeping with our commitment of providing efficient and effective storage and transport of your cargo,
Zip Logistics strongly recommends electing our cargo insurance services which provides coverage for
damaged or lost items during transit.

If no insurance coverage is desired, carefully read and sign this waiver form.

Waiver Agreement:
 Cargo without insurance does not fully protect your interest from losses
 Bill of Lading terms and conditions contains limitations of liabilities
 Following individual/company has waived the recommended insurance and accepts full
responsibility for all possible damage and loss that may occur during transit.
o Further, I/We fully understands that in the event that any damage occurs, Zip
Logistics will not be held liable.
