DATA PRIVACY AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This DATA PRIVACY AGREEMENT is made, executed and entered into by:

____________________________________, of legal age, single, and a

resident of _________________________________________________________
herein referred to as the “CONTRACTOR”

-AND-

XXX COMPANY (Company for brevity), a corporation formed and

organized under Philippine laws, represented by
____________________________, its President, Single Stockholder, and Sole
Director, likewise of legal age, married, and a resident of Narcisa Hills
Subdivision, Rawis, Calbayog City, herein referred to as the “CONTRACTEE”

WITNESSETH;

WHEREAS, the CONTRACTOR is offering his/her accounting services to the

CONRACTEE;

WHEREAS, the CONTRACTEE has agreed to the services offered by the

CONTRACTOR;

WHEREAS, to allow complete access to essential information concerning the

determination of the complete and actual income of the CONTRACTEE, the
CONTRACTEE must share confidential information to the CONTRACTOR;

NOW THEREFORE, in order to protect the CONTRACTEE’s right to privacy

over all information which the CONTRACTOR may obtain, the latter hereby
agrees to be bound by the following terms, to wit:
 1. The CONTRACTOR shall keep confidential ALL information he/she
may be given access to in order to determine the CONTRACTEE’s complete and
actual income;

2. That in case the CONTRACTOR fails to maintain confidentiality of said

information, the CONTRACTEE shall have the right to collect civil indemnity, in
the form of liquidated damages, amounting to

3. That the CONTRACTOR shall immediately surrender all documents

belonging to the CONTRACTEE upon the completion of the service for which
he/she was hired.
