Scenario:

At the night of February 22, 2018, during heavy rain V1 was travelling
Elemento Street while V2 was traversing Bautista Street. Upon reaching
the intersection, said vehicles came into contact. As a result of impact V2
turned turtle.

After the outcome of collision, the driver of V1 left the place of

incident. However, the driver of V2 died on the spot; his passenger
sustained injury and was rushed to the nearest hospital.

During the course of follow – up driver of V1 was found inside room

303 of St. Lukes Hospital due to injuries he sustained and confined thereat
for further treatment. The District Director directed your Chief of Office to
brought the driver for inquest proceeding.
