SUBJECT : Incident Report re Stabbing Incident that resulted to the


Death of Jack Alaska y Tatlonghari

DATE : April 25, 2008

1. On April 25, 2008, at about 10:00 o’clock in the evening, a stabbing


incident transpired in front of Uhaw Sari-Sari Store, located at Nr. 357 Malalim Street,
NAWASA Village, Barangay Dagat-Dagatan, Bacoor, Cavite, wherein one Jack Alaska
y Tatlonghari, 30 years old, single, “balut” vendor, with postal address at Nr. 380
Dimalanta Street, Barangay Diligan, Bacoor, Cavite was stabbed by one Pedro
Dimayuga y Tugatog, 35 years old, married, owner of Uhaw Sari-Sari Store. After the
incident, the suspect hurriedly boarded a bicycle and fled towards the direction of Naval
Street. A neighbor identified as Daniel Hapis y Awa, 30 years old, single, with postal
address at Nr. 359 Malalim Street, NAWASA Village, Barangay Dagat-Dagatan, Bacoor,
Cavite witnessed the incident and hurriedly brought the victim to the Family Health Care
Hospital located along Balasa Highway, Barangay Kamot, Bacoor, Cavite but was
pronounced dead on arrival (DOA) by the attending physician, Dr. Fundador San Miguel
y Albatros. The cadaver was moved to the PNP Crime Laboratory in Imus, Cavite for
autopsy.

2. PO2 Julian Makabayan was assigned to conduct investigation on the


case.

SUBJECT : 1st Progess Report re Stabbing Incident that resulted to the


Death of Jack Alaska y Tatlonghari

DATE : April 26, 2008

1. Reference: Incident Report from this Office dated April 25, 2008, with
subject same as above.

2. Above reference pertains to the stabbing incident that transpired on April


25, 2008, at about 10:00 o’clock in the evening, in front of Uhaw Sari-Sari Store, located
at Nr. 357 Malalim Street, NAWASA Village, Barangay Dagat-Dagatan, Bacoor, Cavite,
wherein the victim, identified as Jack Alaska y Tatlonghari was stabbed by the suspect,
identified as Pedro Dimayuga y Tugatog. The victim was pronounced DOA when rushed
to the Family Health Care Hospital.

3. Investigation disclose that the suspect and the victim had a heated
argument on April 20, 2008, at around 9:30 in the evening, when the former saw the
latter peeping through a hole on the side of the store, which was then being attended to
by Anita, 14 years old, single, daughter of Pedro Dimayuga. Jack Alaska managed to
flee before Pedro Dimayuga could grab him. Jack Alaska was heard cursing and
shouting: “MAKUKUHA KO DIN ANG GUSTO KO! BABALIK AKO!

4. At around 10:00 o’clock in the evening of April 25, 2008, while Pedro
Dimayuga was about to close the store, Jack Alaska approached him and started
shouting “MAKAKAGANTI DIN AKO!”, as he pulled out a knife form the “balut” basket.
Wasting no time, Pedro Dimayuga wrestled for possession of the knife and stabbed
Jack Alaska. Fearing that somebody might see him, Pedro Dimayuga hurriedly fled.

5. This is Office is still waiting for the result of the autopsy report.

6. PO2 Julian Makabayan, the Investigator-on-Case, is convincing Anita


Dimayuga and Daniel Hapis to give their respective sworn statements, as he proceeded
to conduct an ocular inspection of the area.

7. Intelligence operatives are now looking for the possible whereabouts of


the suspect although a feeler for his surrender was sent to the undersigned.

2. This pertains to the stabbing incident that transpired at around 10:00


o’clock in the evening of April 25, 2008, in front of Uhaw Sari-Sari Store, located at Nr.
357 Malalim Street, NAWASA Village, Barangay Dagat-Dagatan, Bacoor, Cavite which
resulted to the death of Jack Alaska y Tatlonghari.

3. For information, during the conduct of ocular inspection, PO2 Julian


Makabayan noticed dried stains on the table outside the store, as well as on a piece of
cloth beneath the table, and on the right side of the wooden wall of the store.

4. PO2 Makabayan called for a Crime Scene Investigator to lift the said
stains, which were then brought to the Crime Laboratory for analysis to determine
whether or not they are of human origin, and if they belong to a single source.

2. In connection with the above references, respective sworn statements of


Anita Dimayuga and Daniel Hapis were taken by PO2 Julian Makabayan.

3. Anita Dimayuga y Panatag, 14 years old, single, daughter of espouses


Pedro and Palma Dimayuga, 2 nd year high school student at Bacoor National High
School, went to this station accompanied by her mother on April 26, 2008. Among other
things, she narrated on her statement, that she was attending to their store at around
9:30 in the evening of April 20, 2008 when she noticed somebody peeping at her
through a hole on the side of their store. When she shouted for help, her father came to
her rescue and saw Jack Alaska as the one peeping at her. Before her father could grab
Jack Alaska, the latter managed to run away, while cursing and shouting:
“MAKAKAGANTI DIN AKO!”. At around 10:00 o’clock in the evening of April 25, 2008,
as she was about to enter their house, she noticed somebody approached her father.
When she verified, she noticed that is was Jack Alaska, who was then holding a knife
and pointing the same at her father Pedro Dimayuga. She started to scream when the
duo grappled for possession of the knife, and she was further astonished when she saw
Jack Alaska fell to the ground while holding his stomach. She noticed her father also
clutching on to his side, which was then bloodied.

4. Likewise, Daniel Hapis y Awa, 30 years old, single, with postal address at
Nr. 359 Malalim Street, NAWASA Village, Barangay Dagat-Dagatan, Bacoor, Cavite
went to this station on April 26, 2008 and volunteered his sworn statement. Among
other things, he narrated that he was standing in front of his house when he hear
screams and noticed the commotion at the nearby Uhaw Sari-Sari Store. When he
verified, he saw Pedro Dimayuga and Jack Alaska fighting for possession of a knife.
When he was about to approached the duo, he noticed Jack Alaska stumbled on the
ground, while Pedro Dimayuga holding his right side, and wiping it with a piece of cloth.
He continued that he immediately brought Jack Alaska to the hospital but was
pronounced dead on arrival.

5. This station is still scouring the area for the possible whereabouts of the
fatal weapon.

2. At around 4:30 in the afternoon of April 28, 2008, the undersigned,


together with elements of Station Alert Team proceeded to the Barangay Hall of
Pulutan, Bacoor, Cavite to facilitate and secure peaceful surrender of Pedro Dimayuga,
the suspect in the death of Jack Alaska on April 25, 2008 in front of Uhaw Sari-Sari
Store, located at Nr. 357 Malalim Street, NAWASA Village, Barangay Dagat-Dagatan,
Bacoor, Cavite.

3. The proposed surrender came about when Barangay Chairman Paterno


Maternal called the station, and relaying the information about the presence of the
suspect who wishes to surrender to the authorities.

4. Chairman Maternal readily turned-over the Pedro Dimayuga to the


authorities. After being positively identified by witness Daniel Hapis, the suspect Pedro
Dimayuga was informed, in a language known to, and understood by him, of his
Constitutional Rights. He was then taken into custody and immediately brought to
Cavite Provincial Hospital for medical examination.

5. Dr. Isagani Haplos noticed a wound on the right side of Pedro Dimayuga
which was caused by a sharp object. Attached is a copy of the findings of Dr Haplos.

6. Pedro Dimayuga is now under the custody of this station and undergoing
further investigation and documentation.

2. For information, at around 8:30 this morning, the undersigned, together


with the Investigator-on-Case returned to the crime scene, particularly in front of Uhaw
Sari-Sari Store, located at Nr. 357 Malalim Street, NAWASA Village, Barangay Dagat-
Dagatan, Bacoor, Cavite to verify the allegations of Pedro Dimayuga that he threw the
fatal weapon during the incident underneath the bushes, and so with the cloth that he
used to wipe the blood oozing from his right side underneath their table.

3. During the said re-inspection of the place, the investigator discovered a


knife underneath the bushes, which Pedro Dimayuga identified as then same knife used
the Jack Alaska in attacking him, and eventually caused the victim’s death.

4. Afterwards, a piece of stained clothing was discovered underneath the


table in front of the store. The same piece of cloth was identified by Pedro Dimayuga as
the one he used in wiping the blood oozing from the right side of his body.

5. The pieces of evidence were properly identified, documented, recovered,


marked and labeled. They were sent to the Crime Laboratory for the corresponding
analysis.
