2. WHAT: Shooting incident resulting to the death of Jerome Agda Y Bengkoy

and Anne Santo Agda Y An-An

3. WHERE: T.Claudio Street Tacloban City

4. WHEN: At about 6:10 PM of January 01, 2022, Reported at about 6:15 PM of

same date

5. WHO: Witness one - Ednalyn Honrales, 23 years old (JULY 18, 1998) Single,
a resident of Brgy 25 Paterno Ext. Tacloban City

Witness Two – Sheila Hannah Marie Hayag, 22 years old (September

07, 1999) Single, a resident of Brgy Alang-Alang Leyte.

Witness Three – Fhevie Gatela,24 years old (November 08, 1997)

Single, a resident of Brgy 08. T. Claudio Street Tacloban City.

6. HOW: Initial Investigation conducted shows that the victim was killed inside the car.
On January 01, 2022 at about 06:10PM in the evening a shooting incident transpared at
T.claudio St. Tacloban City, wherein victim Jerome Agda Y Bengkoy 29 years old, married,
Police officer and a resident of Brgy 62- A Sagkahan, Tacloban City and Anne Santo Agda y
An-An 27 years old, married, Businesswoman and a resident of brgy 62-A Sagkahan, Tacloban
City. Was shoot by unknown suspect. As a result, thereof the victims restrained gunshot wound
in the head part of the body and they are both dead on the spot. The suspect is unidentified but
the bullets that the suspect used was turned over to the evidence custodial.

7. The motive of the incident is undetermined and ongoing investigation.
