SUBJECT : Spot Report on Improvise Explosives Device

DATE : August 15, 2018

1. Spot Report on recovered Improvised Explosives Device (IED). At about 10:30 in the
evening 15 August 2018 joint operating elements from RID09, Tukuran PIB, PPSC,
RPSB09 and Tukuran MPS responded on a cellular call from source of Tukuran PIB
informing that a suspected IED place in white sack which allegedly kept by unidentified
remnants of Ansar Al Khalifa (AKP) led by Mohammad Jaafar Maguid alias Kolongo
(killed) at Barangay Militar, Tukuran Sp,, that upon arrival at the said area unidentified
person was seen by the operating elements fleeing towards west direction that
prompted that troops to pursue however lost along the dark area of said place.
Thereafter, EODT of Tukuran PPSC rendered safe procedure and was later found that
the white sack was containing the following such as;

a) Picture of Honorable Mayor Francisvic Villamero

b) 1pc of non-electric blasting cap
c) 11pcs assorted resistor
d) 45pcs assorted capacitors
e) 1 unit landmine fuse
f) 21 LED
g) ¼ kilo of black powder
h) 1 unit fuse assembly of F1 fragmentation grenade
i) 1pc IED circuit
j) 21 meters detonating commercial cord
k) One unit hand held radio
l) 1 roll of gauge wire
m) 17pcs m 16live ammunition
n) 1pc 45 caliber ammunition
o) 10pcs 555 timer
p) 1 empty m16 plastic magazine
q) 1pc 45 caliber ammunition
r) 1 unit soldering iron
 s) 1 unit multi tester AC/DC
t) 1 everyday battery
u) 6 pcs pliers
v) 1 pc electrical tape
w) 1pc scissor

2. All recovered items were under the custody of Tukuran Provincial Police Officer for
proper disposition.

Re: Spot Report on Improvise Explosives Device at
Brgy. Militar, Tukuran Zamboanga Del Sur

AUTHORITY:

a. R.A 9372 March 6, 2007, an act to secure the state and protect our people from
terrorism.
b. R.A 9516 PD no 1866 as amended entitled codifying the laws on illegal/unlawful
possession, manufacture dealing in acquisition or disposition of firearms,
ammunition or explosives or instrument used in the manufacturer of firearms and
imposing stiffer penalties for certain violations thereof: for relevant purposes.
c. Standard operation procedure number CDIDM 2011-008 dated April 7, 2018
conduct of crime scene investigation.

MATTERS INVESTIGATED:

a. Recovered Improvised Explosives Device (IED) through the joint operating element
from RID, Tukuran PIB, Tukuran PPSC, RPSB09 and Tukuran MPS responded on a
cellular call from the source of Tukuran PIB informing that the suspected IED place
in white sack.
b. The suspected IED was allegedly kept by Ansar Al Khalifa(AKP) Mohammad Jaafar
Maguid alias (Kolongo) (killed).
c. The suspected want to terrorize the said place.

FACTS OF THE CASE:

1. It pertain in spot report dated 10 august 2018 the recovered Improvised

Explosive Device (IED) at about 10:30 in the evening at Militar, Tukuran ZDS. The
IED was own by Mohammad Jaafar Maguid alias (kolongo) was killed.
 2. A police officer 3 Cardo Dalisay who was in charge of joint operating element
from RID09.
3. An interview with the witness Reca Blanca, who lives in the said area, saw the
unidentified persons with suspicious act pass by his store.
4. At about 06:30 in the evening on the same date the suspects arrived in the same
said above area. And the call from the source of Tukuran PIB informing the arrival
of suspects.

DISPOSITION:

1. The said suspects planned to seize the said municipality.

2. The case will turned over to the higher headquarters for the further investigation
for this is the threat of the nation.

1. Spot Report on vehicular traffic accident transpire on 12, September 2018 at about 3:42
PM along the National Highway of Prk. Magsaysay, Balongating, Guipos, ZDS, wherein
two(2) vehicles involved; V1- single MC XRM combination of gray and black color with
plate Nr. 50811LR, OR #1021026296, CR #176887852 owned and driven by Gerald Tian,
36 years old, single and a resident of Purok Linaaw, Barangay Luy-a, Tukuran, ZDS, V2-
Single MC Skygo with color combination Black and red with temporary plate #0901-
0212432 driven by Sanny F. Radan, 46 years old, widow, a resident of Barangay Ipil
Heights, Ipil,ZDS. Initial investigation disclosed that both vehicles are travelling in a
different direction V1 going to Margosatubig and V2 going to Dumalinao. Upon reaching
on the accident place V2 was oveshoot due to high speed and bumped/ collided to
vehicle, which resulted them to crash down along the highway. Both riders were
brought immediately to Zamboanga del sur Provincial Hospital for proper and
immediate medication. Driver of V1 sustain light injuries while V2 fatally injured.

JOINT AFFIDAVIT OF FIRST RESPONDENT

We, SPO2 Cardo Dalisay, legal age, married and PO2 Glady Tiana, legal age,
single, both member of the Philippine National Police and presently assigned in Tukuran
Municipal Station, after duly sworn to in accordance with the law do hereby depose and
states:

That on August 15, 2018, at 7:00 in the evening while we are rendering duties at
Tukuran Municipal police station we received a report thru our Hotline number that
there is an arrival of an unidentified persons armed with high powered gun and the
encounter happened. A terrorist led by Mohammad Jaafar Maguid (kolongo) was killed
in an encounter with RID09, Tukuran PIB, PPSC, RPSB09 and Tukuran MPS, however,
Ansar Al Khalifa was lost along in the dark area of the said place. Thereafter, EODT of
Tukuran PPSC rendered safe procedure and was later found that the white sack was
containing the said IED.

Threafter, Tukuran Municipality police conducted manhunt operation at the

same time communicating in the neighboring municipalities to established a check point
for the apprehension of the suspect( Ansar Al Khalifa).

At about 2:45 in the morning August 17, 2018, there’s a call we received from
Sultan Naga Dimapuro Police Station states that while they conducting a check point in
their posting area, they spotted the suspicious person, that’s why they invited the
suspicious person to ask some question. During questioning to the suspicious person,
they found out that he cannot answer the questions straightly. That’s why the officer in
charge who conducted the check point decided to hold the suspicious person in their
custody and called our office for clarification.

Thereafter, at 8:00 in the morning at August 19, 2018 my team went to sultan
Naga Dimapuro Police Station to confirm the said called, and my team positively
confirmed that he was the terrorist that escaped during operation last August 15, 2018.

That we executing this affidavit freely and voluntarily to at least the truthfulness of
the foregoing facts.

MATTERS INVESTIGATED:

a) To determine what is cause of stabbing

b) To interview/question the person with interest Juan Y. Taklob and Peter
R. Pan.
c) To determine whether the evidence gathered match to the suspected
person with interest.

FACTS OF THE CASE:

1. On or about 8:30 o’clock in the evening august 16, 2018 Barangay

Canunan, Guipos, Zamboanga Del Sur reported for the assistance on the
stabbing incident occur. PO2 Gennyse Fernandez was assign to the crime
scene.

2. Upon investigation the incident John Jacob, 20 years old, married, a

resident of Balongating, Guipos, Zamboanga Del Sur, positively pointed
Juan Taklob who do the stabbing of Fernando Poe. And later on, he
admitted that he is under the influence of liquor. That they just intended
to treat Fernando, but the victim resisted that he almost pulled the knife
in my hand, that’s why I have no choice but to stab him.

DISPOSITION:

1. The stabbing was happened one to due to teasing, under the influence
of liquor and resistance of the victim.
2. This case will be turned over to municipal trial court for proper
punishment penalty.
3. The victim Fernando Poe was died to his multiple injuries suffered.

AFFIDAVIT OF WITNESS

I, John Jacob, 25 years of age, single, resident of Purok Paglaum, Canunan,

Guipos, Zamboanga Del Sur, after having sworn in accordance with law, do hereby
depose and state:

01. That I was present and personally saw that accused, Juan Taklob,
committed the crime of homicide against the victim Fernando Poe.

02. That I was working at Zanaida’s Store where the crime committed.

03. That I was the one who stopped the victim Fernando Poe to ignore Juan
Taklob from teasing him.

04. That I tried to stop them when they were already fighting but I was not
able to do such because I fear for my safety.

05. That I saw Fernando Poe stabbed on the left abdomen using a swiss knife.

I executed this affidavit to attest the truthfulness of the foregoing facts and to
support the filing of Criminal Cases against Juan Taklob for violations of MURDER.

1. As pertain in the spot report an Vehicular Accident transpired on 30 May 2018 at about
03:42 along the national highway of Prk. Magsaysay, Guipos, Zamboanga Del Sur, wherein
two (2) vehicles involve; V1- Single MC XRM combination of gray and black color with
plate No. 50811LR, OR No. 1021026296, CR No. 176887852 driven and owned by Gerald

accident place V2 (Mr. Radan) was overshoot due to high speed and bumped/ collided to
vehicle V1 (Mr. Tian), which resulted them to crush down along the highway and both
drivers were brought immediately to Zamboanga Del Sur Provincial Hospital.
2. That around 11:00 o’clock in the morning on the following day, the son of victim identified
himself as Rodly Radan called through cellular phone informing us that his Father (Sanny
Radan) was pronounced dead by his attending physician at exactly 10:15 o’clock in the
morning of May 31, 2018.

IV. DISPOSITION :

Due to high speed of V2 (Sanny Radan) which resulted him to bumped/ collided to V1
(Gerald Tian) and a clearly Reckless Driving resulting
