MEMORANDUM
2. Be informed that at around 5:00 PM of April 28, 2022, PMSg Jun-jun Ledda of Kalinga
Provincial Forensic Unit, turned over to PSSg Buyogan (IOC) one (1) realme cellular phone
color blue and black recovered in the left pocket of one of the slain suspects. A minute later,
unregistered cellular phone number was calling on the said cellular phone was entertained and
able to know the identity of the caller as A.K.A “Ashly” who was looking to a certain name a.k.a
‘Jose’ and Ian Zulueta. She was then informed regarding the incident and death of the
possessor of CP together with his four companions.
3. Personal belongings and identification cards were also recovered inside the used
vehicles are all under the custody of the IOC were as follows;
a. One (1) black belt bag containing wrist watch casio sensor, charger,40 pesos
(2 pcs 20 peso bill), Palawan Pawnshop suki card with number CC 4 248 646, Driver’s
license of ZULUETA, IAN S, vaccination card of ZULUETA, IAN S, photocopy OR/CR of
Mitsubishi Montero owned by Frederico Santos Saludan, medallion and folded knife;
b. One (1) pixelized sling bag containing knuckles and knife; and
c. One (1) realme cellular phone color blue and black recovered in the left
pocket of the Jose M Fajardo;
d. One (1) light brown wallet with prints “GD” in checkered design containing
one (1) pc 100-peso bill, twelve (12) pcs 20-peso bill with total amount of three hundred
forty pesos and assorted IDs under the name of JOSE S. LIBREJA.
4. At around 12:05 AM of April 30, 2022, the relatives of the five (5) suspects arrived and
coordinated with the personnel of Tabuk City Police Station located at Dagupan Centro, Tabuk
City, Kalinga. The duty investigator of Substation Bulanao PSSg Rostigue turn over the
personal belongings of Jose M. Fajardo, Ian S. Zulueta and Jose S. Libreja enumerated above
and proceeded at TAMPCO (Tabuk Multi-Purpose Cooperative) Funeral Services City and
personally identified the five (5) suspects as;
a. Ian Salutin Zulueta, Filipino, male, Barangay Tanod, 37 y/o (DOB: 1985/2/11),
and resident of B-21 L-37 Fuchsia St. Royal Meadows Subdivision, Mexico, Pampanga
claimed by his son Christian Mhiles Yabao Zulueta (minor);
b. Jose Sarro Libreja Jr, Filipino, 36 y/o (DOB: 07/22/1985), male and resident
of B-19 L-5 Lilac St. Royal Meadows Subd., Brgy. Tangle Pampanga with GUARDIANS
tattoo on the left arm and “DOMINIC CO” tattoo at his back claimed by his wife Ma.

5. The relatives of the suspects showed and submitted a copy of Certification from the
Municipal Police Station that they are no longer interested to conduct Autopsy Examination of
their relatives. The five (5) cadaver of the suspects are now being travelled and will be brought
at their own residence at Royal Meadows Subd., Brgy. Tangle, Mexico Pampanga on board
with the Ambulance Funeral Service of Mexico, Pampanga.
6. Also Be informed that the two (2) persons who were dead on the spot later identified
as Mr. Ian Salutin Zulueta (driver) and Jose Sarro Libreja Jr. (with recovered one (1)
COLT cal. 45 with SN 743961 near him).
7. The other three (3) suspects who were brought at Mejia Kim Hospital later identified
as Joel Mengote Fajardo (recovered firearm from where he rested before he was brought at
the hospital is one (1) cal. 45 M1911A1 bearing SN 109411); Eric Orquia Acodesin,
(recovered firearm from where he rested before he was brought at the hospital is one (1) cal. 45
COLT MK IV Series 80 cal. 45 with SN 641138) and Terio Orquia Acodesin, (recovered
firearm from where he rested before he was brought at the hospital is one (1) cal. 45
ARMSCOR with SN 288207).
8. At around 8:00 AM of April 30, 2022, the IOC of the Tabuk CPS conducted
background investigation and based on the CIRAS (Crime Information Reporting and Analysis
System) three (3) of the suspects namely; Eric Orquia Acodesin, Terio Orquia Acodesin and
Joel Mengote Fajardo are drug surrenderee (TOKHANG) on December 2016 at Mexico,
Pampanga;
e) Based on the Chemistry report (GPR Examination) of the four (4) cal. 45,
COLT MK IV Series 80 cal. 45 with SN 641138 marked as JOL-13 4-28-22, COLT cal.
45 M1911A1 with SN 109411 marked as JOL -17 4-28-22, COLT cal. 45 M1911A1 with
SN 743961 marked as JOL-1 4-28-22, ARMSCOR cal. 45 with SN 288207 marked as
JOL-6 4-28-22, the qualitative examination conducted on the said specimens gave
positive result to the test for the presence of gun powder nitrates;
f) Based on the result of Paraffin Test on both hands of the five (5) cadaver,
qualitative examination conducted gave negative result to the test for the presence of
gun powder nitrates;
g) Based on the Macro-etching Examination of the four (4) firearms, the three
(3) cal. 45 gave a conclusion that there SN is not tampered except the one (1) Cal. 45
Colt M1911A1 with SN 743961 marked as JOL-1 4-28-22 which has the presence of
alteration on the fourth digit number 9 superimposed by number 8. 5th Progress Report
