1 Properly filled-up Loan Application Form

2 Properly filled-up Customer Information Sheet
3 Certification from OWWA that the borrower is a bonafide overseas
worker
4 Certification from OWWA that the OFW-borrower has completed
capacity-building training
5 For start-up project - Business Plan or Feasibility Study
6 DTI Registration
7 Mayor's Permit
8 BIR Certificate of Registration
9 Marketing Agreement
10 Other Permits (If applicable)
a) NFA License
b) FPA License
c) BFAD License
11 Audited Financial Statements for the last 3-years (BIR Filed)
12 Interim Financial Statements for the immediately preceding calendar
quarter (for loan applications filed after March 31)
13 Income Tax Return (last 3 years)
14 Pictures of Business Site
15 Environmental Compliance Certificate (ECC) or Certificate of Non-
Coverage (CNC) whichever is applicable.
16 Discharge permit/Permit to Operate from DENR-EMB, if applicable
17 Standard documentary requirements such as evidences of ownership
of offered collateral:
a) Register of Deeds authenticated copy of land Title
b) Tax Declaration
c) Real estate tax clearance and tax receipt for the current year.
d) Subdivision plan approved by the Land Management Bureau-DENR
e) Sketch Plan with vicinity map
f) Affidavit of Non-tenancy for agricultural lands (if applicable)

a) Cost Estimates/Bill of Materials
b) Project plans and specifications
c) PERT-CPM
