SENSORY EVALUATION FORM
INSTRUCTIONS
1. Evaluate the control sample for appearance, aroma and taste to familiarize yourself with the
product sensory characteristics.
2. Evaluate the sample for appearance, aroma and taste.
3. The test sample is considered OUT if it is found to be uncharacteristic or has an off note as
compared to the control sample while the test sample is considered IN if its characteristics
match the control sample.
