The Petitioner, thru the Public Attorney’s Office, unto this Honorable Court,
most respectfully offers in evidence the following exhibits:

EXHIBIT NATURE and DESCRIPTION

“A” Petition dated May 7, 2019

Purpose: Exhibit “A” and its sub-markings is formally offered as part of the
testimony of the Petitioner to prove that the Petitioner has filed a Petition
before the Court which has jurisdiction to hear and decide the petition.

“B” Certificate of Indigency issued by Barangay

Tinampaan, Cadiz City

“C” Certificate of Indigency issued by CSWD of Cadiz

City

“D” Affidavit of Indigency in the Client’s profile of

the Public Attorney’s Office of Cadiz City

Purpose: Exhibits “B”, “C” and “D” and their sub-markings are formally
offered to prove that Petitioner is an indigent and is entitled to the legal
services of the Public Attorney’s Office and that in accordance with the
provisions of Republic Act 9406 and its implementing rules and regulations,
petitioner is exempt from paying the docket and other legal fees incidental
and subsequent to the filing of the petition.

“E” Order of the Honorable Court dated June 17,

Purpose: Exhibit “E” is formally offered to prove that the Honorable Court
gave due course to the petition and set it for hearing on September 9, 2020.

“F” Notice of Raffle

Purpose: Exhibit “F” with its sub-markings is formally offered to prove that a
Notice of Raffle which directs the Petitioner to publish the Order in the
NEGROS WEEKLY, a newspaper of general circulation in the province.

“G” July 19-25, 2020 issue

“H” July 26 - August 1, 2020 issue

“I” August 2-8, 2020 issue

Purpose: Exhibits “G”, “H” and “I” and their sub-markings are formally
offered to prove that the petition was published in three (3) consecutive
weeks in NEGROS WEEKLY, a newspaper of general circulation.

“J” Affidavit of Publication

Purpose: Exhibit “J” and its sub-markings is formally offered to prove that
the publication was made by the publisher of NEGROS WEEKLY in its July 19-
25, 2020 issue, July 26 - August 1, 2020 issue, and August 2-8, 2020 issue.

“K” Notice of Appearance of the Solicitor General

Purpose: Exhibit “K” and its sub-markings is formally offered to prove that
the Solicitor General has entered its appearance for and in behalf of the
government in the hearing of this petition.

“L” Deputization of the Solicitor General deputizing

the City Prosecutor’s Office of Cadiz City

Purpose: Exhibit “L” and its sub-markings is formally offered to prove that
the Solicitor General has deputized the City Prosecutor of Cadiz City to appear
for and in behalf of the government in the hearing of this petition.

“M” Certificate of Live Birth Registry No. 94-3248

Purpose: Exhibit “M” and its sub-markings is formally offered to prove the
facts of birth of the petitioner.

“N” Baptismal Certificate

Purpose: Exhibit “N” and its sub-markings is formally offered to prove that
the name of petitioner is JOHNCY A. MARGAHA.

“O” Elementary School Permanent Record

Purpose: Exhibit “O” and its sub-markings is formally offered to prove that
the petitioner is using the surname MARGAHA.

“P” High School Permanent Record

Purpose: Exhibit “Q” and its sub-markings is formally offered to prove that

Purpose: Exhibit “R” and their sub-markings are being offered to prove that
petitioner is using the surname MARGAHA.

“S” Marriage Contract Registry Number 93-647

Purpose: Exhibit “S” and its sub-markings is formally offered to prove that
EUGENIO MARGAHA is married to MARIA ALVAREZ.

“T” Certification from the Office of the Civil

Registrar General

Purpose: Exhibit “T” and its sub-markings is being offered to prove that
Maria Alvarez has a previous marriage with Isidro Layese.

“U” Affidavit of Acknowledgment and Admission of

Paternity

Purpose: Exhibit “U” and its sub-markings is being offered to prove that
Heracleo Bansag acknowledged the petitioner as his own son.

“V” Affidavit to use the surname of the father

Purpose: Exhibit “V” and its sub-markings is being offered to prove that
petitioner is wishes to use the surname of his father.

“W” Judicial Affidavit of Johncy Alvarez

Purpose: Exhibit “W” and its sub-markings is being offered as the direct
testimony of Johncy Alvarez.

“X” Judicial Affidavit of Maria Alvarez Margaha

Purpose: Exhibit “Z” and its sub-markings is being offered as the direct
testimony of Maria Alvarez Margaha.

PRAYER

WHEREFORE, it is most respectfully prayed of the Honorable Court that the

foregoing exhibits be admitted for purposes they are being offered.

Other just and equitable relief under the circumstances are also prayed for.
