PERINEUM- is the area between the


thighs and from the anterior pelvis
to the anus.

Female- it is the area between vagina


and rectum.
Male- it is between the scrotum and
anus.

- is a hygienic care that


involves cleaning the
perineum and genitalia.

PURPOSES:
• To remove normal perineal
secretions and odors

• To promote client comfort

• prevent infection or
contamination from the rectum

• patient in labor and post partum.

• Gynecological patient’s with perineal


repair.
• Comatose

• Diabetic Patient
• Patient with indwelling catheter

PROCEDURE / RATIONALE:
1.Prepare all the needed materials.
Rationale: To save time
and effort.

2. Prior to performing the procedure,
introduce self and verify the client’s
identity using agency protocol.
Explain to the client what you are
going to do, why it is necessary

Position the patient on dorsal recumbent (back-
lying position with the knees flexed and spread
well apart)

Cover her body and legs with the bath blanket
positioned so a corner is at her head, the
opposite corner at her feet , and the other two
on the sides.

Place the rubber sheet/draw sheet and bed
pan under the patient's buttocks .

RATIONALE:

soiled.

6. Inspect the perineum then flush the perineal
area with warm water.

Rationale:
Note particular areas of inflammation, swelling
excessive discharge or secretions from the
orifice and presence of odor. Warm water
is used to prevent chilling and soften or
dissolve blood clots.

7. Get 7 pcs of cotton balls soaked in a
cleansing solution using pick up forceps.
Rationale: To maintain sterility.

8. Cleanses external genitalia starting from
midline of symphysis pubis down to the anus.
Never retrace a stroke.

Rationale: To prevent contamination of


the area.

9. With the second cotton ball cleans starting
from mons pubis in figure of 7 by way of external
labium towards the anus. Discard.

RATIONALE:
Secretions that tend to collect around the labia
minora facilitate bacterial growth

10. Do likewise on the opposite side with the
next cotton ball.

11. With the fourth cotton ball cleans starting


from mons pubis in figure of 7 by way of
internal labium towards the anus. Discard.

12. Do likewise on the opposite side with the next


cotton ball.

13. With another cotton ball, cleans groin-starting


from groin going up the thigh . Use another
side of the same cotton ball for near groin.

14. Flush perineum thoroughly with sterile warm


water.

15. Using 7 dry cotton balls, dries the perineum


with the same stroke as above.(procedure #8-
#13)

Rationale: Moisture support the growth of


many microorganism.

16. Remove the bedpan gently and turn the


patient to side.

17. If necessary, takes one cotton ball and


cleans one side of the buttocks. Then clean
the other side of buttocks and dry both
buttocks.
Rationale: To provide comfort.

18. Turn patient on her back and make her


comfortable.
19. Sprays with antiseptic as needed.
Rationale To promote healing.
20. Puts the perineal pad with an updown
motion, as needed.
Rationale To prevent contamination of
vagina and urethra from anal area.

21. Does after care of the materials used.
Remove and discard gloves. Perform hand
hygiene.
22. Record the treatment done to patient and
any unusual findings such as redness ,
excoriation, skin breakdown, discharge or
drainage, and any localized areas of
tenderness.

For documentation purposes that procedure
was done and means of communication.
